摘 要:目的:探讨低分子肝素(LMH)抗凝治疗对慢性肺源性心脏病失代偿期(LCPCPHD)血清D-二聚体(D-D)、凝血因子Ⅷ相关抗原(ⅧR:Ag)及凝血因子Ⅷ(Ⅷ:C)的影响.方法:将70例LCPCPHD患者按就诊顺序1:1比例随机分成观察组和对照组,各35例,两组患者均给予常规综合治疗,包括:吸氧、解痉平喘、止咳化痰、抗感染,活血化瘀及扩血管等治疗,观察组在此基础上加用LMH 5000 u,1次/d.两组均以10d为一个疗程.观察两组D-D、ⅧR:Ag和Ⅷ:C治疗前后变化.结果:①观察组总有效率为91.4% (32/35),对照组总有效率为74.3% (26/35),两组比较差异有统计学意义(P<0.05);②两组治疗后血清D-D、ⅧR:Ag及Ⅷ:C浓度均明显下降,观察组下降更明显,与对照组相比差异均有统计学意义(P<0.01);③观察组治疗后血清D-D、ⅧR:Ag及Ⅷ:C浓度均明显下降,与治疗前对比差异均有统计学意义(P<0.01).结论:使用LMH抗凝治疗LCPCPHD可降低血液的高凝状态,提高疗效.OBJECTIVE: To explore the influence of low molecular heparin (LMH) anticoagulant therapy on serum D - dimer (D- D), factor Ⅷ related antigen (Ⅷ R: Ag) and factor Ⅷ (Ⅷ: C ) chronic pulmonary heart disease loss compensatory stage (LCPCPHD). METHODS: 70 patients with LCPCPHD according to 1:1 ratio of the visiting sequence were randomly divided into observation group (n = 35 ) and control group (n = 35 ). The two groups were given conventional treatment, including oxygen, spas- molysis, relieving cough and phlegm, antiinfection, activating blood circulation to remove stasis and vasodilator therapy. And obser- vation group used LMH based in the therapy above (5000U, 1 times daily). In the two groups 10 days were as a course of treatment. The changes of D - D, ⅧR: Ag andⅧ: C before and after treatment in the two groups were observed. RESULTS: ①The total ef- fective rate in the observation group was 91.4% (32/35) and 74. 3% (26/35) in the control group. There was statistically signifi- cant difference in the two groups (P 〈 0. 05 ) ; ②After the treatment in the two groups serum D -D, ⅧR: Ag and Ⅷ: C concentra- tions were significantly decreased, the ones in observation group decreased more significantly. There were statistically significant differences (P 〈 0.01 ) ; ③In the observation group after treatment, serum D -D,ⅧR: Ag and Ⅷ: C concentrations were signifi- cantly decreased compared with the ones before treatment. There were statistically significant differences ( P 〈 0.01 ). CONCLU- SION: LCPCPHD can reduce the high blood coagulation state and improve the curative effect by using LMH.
