顽固性血精32例MRI检查影像学特征分析

摘　　要：目的：分析顽固性血精MRI检查的影像学特点,探讨MRI在顽固性血精诊断中的价值。方法：回顾性分析2013年1月～2015年4月我院收治32例顽固性血精患者的临床资料,分析其影像学特点。结果：所有患者均完成MRI检查,发现精囊内信号异常17例;单侧或者双侧精囊扩张14例;精囊扩张合并精囊内信号异常8例,发现前列腺小囊囊肿2例,苗勒氏管囊肿1例,射精管囊肿2例;盆腔内精囊区域无明显改变4例。结论：MRI可清楚显示精囊、前列腺及射精管的精细结构改变,对顽固性血精出血部位及出血原因的诊断具有重要价值。顽固性血精MRI检查主要表现为精囊和射精管的扩张、精囊内信号异常以及精囊区域的囊性改变。Objective：To evaluate the diagnostic value of magnetic resonance imaging（MRI）in persistent and refractory hematospermia by analyzing pelvic MRI manifestations and characteristics.Method：The clinical data and morphological features of pelvic MRI of 32 patients with persistent and refractory hematospermia from Jan.2013 to Apr.2015 were retrospectively analyzed.Result：All patients with persistent and refractory hematospermia underwent pelvic MRI examination.Seventeen patients showed the typical changes of signal intensity in seminal vesicle,14 patients showed cystic dilatation of unilateral or bilateral seminal vesicles without the internal signal intensity change,eight patients showed significant cystic dilatation of seminal vesicles with the internal signal intensity change,two patients showed prostatic utricular cysts,one patient showed Mullerian cysts,two patients showed ejaculatory duct cysts and four patients did not show any obvious abnormality in seminal vesicle area.Conclusion：MRI had great value in the diagnosis of persistent and refractory hematospermia,as it can clearly display the pelvic soft tissues and refined structures.The main imaging features of persistent and refractory hematospermia in MRI were changes of signal intensity in seminal vesicle,cystic dilatation of seminal vesicles with or without the internal signal intensity change and cystic changes in seminal vesicle area.
