摘 要:目的探讨右美托咪定应用于脑血管瘤手术对S100β蛋白、神经元特异性烯醇化酶(NSE)影响,评估其脑保护作用。方法从医院2014年1~6月的患者中选取48例需行脑血管瘤手术治疗患者入选本研究,ASA分级为Ⅰ~Ⅲ级,按照数字表法随机均分为2组,观察组诱导时给予右美托咪定1μg·kg^(-1),泵注时间10min,然后以0.5μg·kg-1·h^(-1)的速度泵注;对照组给予生理盐水,比较2组患者血流动力学变化及S100β蛋白、神经元特异性烯醇化酶水平变化。结果观察组给予右美托咪定后,心率、平均动脉压均更为平稳,P<0.05。观察组血清S100β蛋白T5,T6,T7和T8分别为(1.52±0.35),(1.69±0.33),(1.72±0.42)和(2.08±0.57)μg·L^(-1),均低于对照组,P<0.05。观察组血清NSE水平T5,T6,T7和T8分别为(10.9±1.2),(13.5±1.5),(16.8±2.8)和(19.7±4.3)ng·L^(-1),均低于对照组,P<0.05。结论右美托咪定应用于脑血管瘤手术有助于血流动力学稳定,且S100β蛋白、神经元特异性烯醇化酶水平更低,具有重要的脑保护价值。Objective To investigate the effects of dexmedetomidine on S100βprotein,neuron specific enolase(NSE)level in brain aneurysm surgery,then to investigate the cerebral protective effect.Methods 48 cases of patients with intracranial aneurysm treated in the hospital from January 2014 to June 2014 were selected and randomly divided into two groups.The observation group adopted 1μg·kg^(-1) dexmedetomidine when induced with 10 min infusion time,then the group adopted 0.5μg·kg^(-1)·h^(-1) speed pump injection;the control group were given normal saline.Then the hemodynamic changes of S100βprotein,neuron specific enolase(NSE)level were compared.Results The heart rate and mean arterial blood pressure were more stable in the observation group after dexmedetomidine treatment,P〈0.05.The serum S100 beta protein in T5,T6,T7 and T8of observation group were(1.52±0.35),(1.69±0.33),(1.72±0.42)and(2.08±0.57)μg·L^-1 and were decreased than that of the control group,P〈0.05.The serum level of NSE of T5,T6,T7 and T8of the observation group were(10.9±1.2),(13.5±1.5),(16.8±2.8)and(19.7±4.3)ng·L^-1,and were lower than that of the control group,P〈0.05.Conclusion Dexmedetomidine for cerebral aneurysm operation is helpful to maintain the stability of hemodynamics,and the levels of S100βprotein,neuron specific enolase were decreased,which is meaningful for cerebral protection.
