摘 要:目的:了解新生儿产ESBLs菌感染情况并探讨护理措施。方法对2013年1月至2015年12月住院的1112例新生儿于入院当天和入院72 h分别采集粪便标本进行产ESBLs菌检测。将患儿一般资料、产ESBLs菌检测结果进行统计分析;对住院新生儿产ESBLs菌感染者进行各项消毒和前瞻性的隔离。结果入院当日粪便细菌阳性检出率为44.51%,其中产 ESBLs 菌占48.08%;入院当日粪便产 ESBLs 菌阳性检出率为21.40%(238/1112)。入院72 h后阳性检出率为84.62%,其中产ESBLs菌占53.45%;入院72 h后粪便产ES-BLs菌阳性检出率为45.23%(503/1112)。2013年、2014年和2015年院内感染率分别为41.86%(234/559)、42.54%(114/268)和34.39%(98/285),其中产ESBLs菌占比分别是70.09%、56.14%和37.76%。结论新生儿存在产ESBLs菌院内感染,针对性采取前瞻性隔离的护理措施可降低新生儿产ESBLs菌感染率。Objective To understand the incidence of ESBLs-producing bacteria infection and explore the nursing measures. Methods A total of 1 112 cases of newborn babies hospitalized from January 2013 to December 2015 in our hospital were selected as the research objects. Faeces samples taken on the date of admission and 72 hours after the admission were sampled to conduct the ESBLs bacteria detection. The neonate's general information and the results of ESBLs bacteria detection were analyzed; the hospitalized neonates with ESBLs bacteria infection were disinfected and isolated. Results The positive detection rate of faeces taken on the day of admission was 44. 51%, among which the ESBLs producing bacteria accounted for 48. 08%; the ESBLs producing bacteria 's positive detection rate of faeces on the day of admission was 21. 40% (238/1 112). The ESBLs producing bacteria's positive detection rate of faeces 72 hours after admission was 45. 23% (503/1 112). The nosocomial infection rate in 2013, 2014 and 2015 was 41. 86%(234/559), 42. 54% (114/268) and 34. 39% (98/285) respectively, among which the ESBLs producing bacteria accounted for 70. 09%, 56. 14% and 37. 76% respectively. Conclusion ESBLs nosocomial infection existed among the neonates and neonatal ESBLs producing bacteria infection can be reduced by taking prospective isolation nursing in-tervention.
