摘 要:目的:观察解脲支原体(UU)感染对精子形态、顶体酶活性、精液白细胞的影响。方法:取269例男性不育病人精液,根据精液解脲支原体(UU DNA)检测结果,分为UU阳性组(81例)、UU阴性组(188例),依据WHO 精液分析手册,采用WLJY-9000型精液分析系统检测精液白细胞、Diff-Quik染色法分析精子形态、改良Kennedy法测定顶体酶活性,分析解脲支原体(UU)感染对精子形态、顶体酶活性、精液白细胞的影响,采用精子染色质扩散(sperm chromatin dispersion,SCD)方法检测精子DNA 损伤,探讨解脲支原体(UU)感染与精子DNA损伤的相关性。结果:与解脲支原体阴性组比较,阳性组精子形态畸形率特别是头部畸形率显著增高(P〈0.05)、精液白细胞异常显著增高(P〈0.05)、精子顶体酶活性显著降低(P〈0.05),精子DNA损伤升高,但无统计学差异(P〉0.05)。结论:解脲支原体感染可导致精子形态畸形率特别是头部畸形率显著升高,精液白细胞显著增高、精子顶体酶活性显著降低,从而导致男性不育率增高;解脲支原体感染与精子DNA损伤之间呈现正相关趋势,但未显示统计学差异,可能与样本数量有关。Objective:to study the effect of Ureaplasmaurealyticum (UU) infection on sperm morphology, acrosin activity, and white blood cell (WBC) count in semen.Methods:Semen was collected from 269 male patientswith infertility and assigned to the following two groups based on the result of UU detection : UU positive (n=81)and UU negative (n=188).All semen analysis in this study were performed according to WHO laboratory manual for the examination and processing of human semen. WBC in semen was measured using a computer-automated semen analysis system (CASA) (model: WLJY-9000);sperm morphology was determined by Diff-Quik staining; acrosin activity was detected using modified Kennedy method. The association between UU infection and sperm DNA damage was then analyzed based on these results.Results:Compare with the UU negative group, sperm abnormal rate is significantly higher in UU positive group (P〈0.05); In addition, a dramatically increased WBC count (P〈0.05)and decreased acrosin activity (P〈0.05)were also observed in the UU positive group. Although sperm DNA damage also showed a trend of increase in the UU positive group, it isnot statistically different from the UU negative group (P〉0.05).Conclusions: the above results indicate that UU infection can lead toabnormal sperm morphology,especially sperm head deficit, increased white cell count, and inhibitedactosin activity, all of which are probably the mechanisms underlyingmale infertility. Although the positive correlation between UU infection and sperm DNA damage did not reach statistical significance, it could be due to the limited sample size in this study.
