Treg细胞和IL-12在肺结核合并老年慢性支气管炎患者外周血中的表达及意义

摘　　要：目的研究CD4^+CD25^+CD127^-调节性T细胞(T regulatory cell,Treg)与白细胞介素12(interleukin-12,IL-12)在肺结核合并老年慢性支气管炎(老慢支)患者外周血中的表达变化及临床意义。方法收集河源市人民医院2012年1月至2015年6月新发肺结核合并老慢支的病人80例(观察组),并以同期单纯老慢支病人80例作为对照组1,健康体检人群80例作为对照组2。分别在治疗前、治疗3月末和6月末,抽取外周血,采用酶联免疫吸附试验(enzyme linked immunosorbent assay,ELISA)检测IL-12水平,流式细胞术检测Treg细胞的表达。采用SPSS 17.0软件,配对t检验比较各组间差异。结果治疗前,观察组患者外周血IL-12水平显著低于两组对照组(P<0.05);CD4^+T细胞值低于两组对照组(P<0.05),Treg细胞水平高于两组对照组(P<0.05),差异均具有统计学意义。治疗3、6个月末,观察组患者外周血调节性T细胞水平明显降低,而IL-12表达水平上升(P<0.05),差异均具有统计学意义。结论外周血Treg细胞和IL-12表达水平与肺结核具有相关性,可以作为鉴别诊断、病情转归的的辅助指标。Objective To investigate the expression of CD4^＋CD25^＋CD127^-regulatory Tregulatory cell（Treg） and interleukin-12（IL-12）in the aged pulmonary tuberculosis complicated with chronic bronchitis.Methods 80 cases of aged patients with active pulmonary tuberculosis complicated with chronic bronchitis were included in an observation group from January 2012 to December 2015 in the Peoples Hospital of Heyuan city. 80 cases with simple old chronic bronchitis and 80 healthy people from physical examination were chosen as the control group 1 and group 2, respectively. The levels of Treg cell（CD4^＋CD25^＋CD127^-）ratio and peripheral blood IL-12 of the observation group were determined in 0,3 and 6 month intervals and compared with the control groups. Results IL-12 level of observation group was significantly lower than the control groups before treatment,the difference was statistically significant（P〈0.05）,and CD4^＋T cells ratio in the peripheral blood is lower than the control groups（P〈0.05）,meanwhile the level of Treg cells is higher than the control groups（P〈0.05）. Level of Treg cells decreased significantly and the expression of IL-12 increased after3,6 month treatment（P〈0.05）. Conclusion Expression of Treg cells and IL- 12 is associated with the aged pulmonary tuberculosis complicated with chronic bronchitis,and can be used as a indicators of differential diagnosis and clinical outcome of the disease.
