射频消融术治疗小儿心律失常的要点分析及临床观察

摘　　要：目的研究射频消融术对小儿心律失常的治疗效果。方法选取2012年7月至2015年7月本院就诊的158例心律失常患儿为研究对象,经心电生理学检查确定心律失常类型并行射频消融术。观察并比较不同类型、不同年龄段患儿的手术成功率、复发及并发症发生情况。结果 158例心律失常患儿中,房室折返性心动过速93例（58.86%）,房室结内折返性心动过速36例（22.78%）,室性心动过速14例（8.86%）,局灶性房性心动过速10例（6.33%）,切口折返性房性心动过速5例（3.16%）。158例患儿手术成功率为94.94%,复发率为6.00%。其中局灶性房性心动过速患儿复发率（22.22%）最高,与其他各类型患儿比较差异均具有显著性（P〈0.05）。1~3岁患儿手术成功率明显低于其他各年龄段患儿,平均手术时间明显长于其他各年龄段患儿（P〈0.05）。结论射频消融术治疗小儿心律失常,成功率高,复发率低,对于有适应证的心律失常患儿可作为首选治疗手段。Objective To study the efficacy of radiofrequency ablation in pediatric arrhythmia. Method From July 2012 to July 2015, 158 pediatric arrhythmia children in our hospital were chosen as research objects. All of the children underwent cardiac electrophysiology procedures to determine the type of arrhythmia and radiofrequency ablation. Observed and analyzed the success rates, recurrence rates after treatment in children with different arrhythmia types and different age, as well as the complications. ResultAmong 158 children, 93 cases（58.86%） with atrioventricular reentrant tachycardia, 36 cases（22.78%） with atrioventricular nodal reentrant tachycardia, 14 cases（8.86%） with ventricular tachycardia, 10 cases（6.33%） with focal atrial tachycardia and 5 cases（3.16%） with incisional reentrant atrial tachycardia. The total success rate of arrhythmia ablation was 94.94%, and the recurrence rate was 6.00%. The recurrence rate of focal atrial tachycardia was the lowest（22.22%）, which were significantly different from all the other types（P〈0.05）; the success rate of radiofrequency ablation for children ages 1 ~ 3 was significantly lower than other age groups, and average operative time was significantly higher than the other age groups（P〈0.05）. Conclusion The application of radiofrequency ablation in pediatric arrhythmia have high success rate, low recurrence rate. It can be used as the preferred treatment in pediatric arrhythmia with indication.
