摘  要:目的探究血清γ-痕迹蛋白(Cys-C)及总胆汁酸(TBA)在判断新生儿ABO溶血病(ABOHDN)患儿肝肾功能损害中的价值。方法选取2011年8月至2015年8月本院收治的ABO-HDN新患儿55例纳入观察组,另选取同期55例健康新生儿纳入对照组。检测两组新生儿血清Cys-C、TBA、β2-微球蛋白(β2-MG)、总胆红素(TBIL)、谷丙转氨酶(ALT)、谷草转氨酶(AST)、尿素氮(BUN)、血肌酐(Scr)水平,并分别分析Cys-C、TBA、β2-MG水平与TBIL、ALT、AST、BUN、Scr水平的相关性。比较Cys-C、TBA、β2-MG检测ABO-HDN的灵敏度和特异性。结果观察组新生儿各检测指标水平均明显高于对照组(P<0.05)。观察组新生儿Cys-C、TBA、β2-MG水平与TBIL、ALT、AST、BUN、Scr水平均呈正相关(P<0.05)。观察组55例新生儿中,48例肝功能损害,50例肾功能损害。在ABO-HDN患儿肾功能损害中,Cys-C检测灵敏度为84.0%,特异性为60.0%;β2-MG检测灵敏度为70.0%,特异性为40.0%。在ABO-HDN患儿肝功能损害中,TBA检测灵敏度为83.3%,特异性为71.4%;ALT检测灵敏度为68.8%,特异性为42.9%;AST检测灵敏度为70.8%,特异性为57.1%。结论血清Cys-C与TBA在判断ABO-HDN患儿肝肾功能损害中较为敏感,有助于ABOHDN患儿肝肾功能损害的早期诊断和治疗。Objective To explore the value of setrum Cystatin C(Cys-C) and total bile acid(TBA) in the jugement of liver and kidney function damage in ABO hemolytic disease of the newborn(ABO-HDN). Method Selected 55 ABO incompatibitily newborn accepted by our hospital from August 2011 to August 2015 as observation group, and selected 55 healthy children as control group. The value of Cys-C, TBA, β2-MG, TBIL, ALT, AST, BUN, Scr were detected and individual analysis correlation between Cys-C, TBA, β2-MG and TBIL, ALT, AST, BUN, Scr were taken. The sensitivity and specific degrees of ABO-HDN were detected according to Cys-C, TBA, β2-MG. Result The indexes of observation group were obviously higher than control group(P〈0.05). It was showed in correlation analysis that positive correlation between Cys-C, TBA, β2-MG and TBIL, ALT, AST, BUN, Scr(P〈0.05). It was showed in sensitivity analysis and specific analysis that, during the liver damage study of ABO-HDN, sensitivity detected of Cys-C was 84.0%, the specific detected was of 60.0%; sensitivity detected of β2-MG was 70.0%, the specific detected was of 40.0%. During the liver damage study of ABO-HDN, sensitivity detected of TBA was 83.3%, the specific detected was of 71.4%; sensitivity detected of ALT was 68.8%, the specific detected was of 42.9%; sensitivity detected of AST was 70.8%, the specific detected was of 57.1%. Conclusion The setrum Cys-C and TBA are sensitive in the jugement of liver and kidney function damage in ABO hemolytic disease of the newborn. It means a great deal to clinic.
