2011年福建省流脑流行期两地市健康人群带菌率及抗体水平分析

摘　　要：目的了解2011年福建省健康人群流脑带菌率及抗体水平,为制定防控措施提供依据。方法采集不同年龄组健康人群的咽拭子及血清标本,用ELISA法检测血清抗体,E-test法进行药敏试验,SPSS 17.0软件包分析。结果 2011年咽拭子标本中未检出脑膜炎奈瑟球菌（0/263）;流脑抗体总阳性率为87.33%（324/371）,流脑A群和C群抗体阳性率分别为78.98%（293/371）和66.04%（245/371）;A群以1~3岁年龄组抗体阳性率最高（87.76%,43/49）,3~5岁年龄组最低（73.47%,36/49）;C群以≥20岁年龄组抗体阳性率最高（74.19%,46/62）,3~5岁年龄组最低（51.02%,25/49）。龙岩市流脑A群抗体阳性率和浓度均高于屏南县（阳性率分别为87.50%、69.01%,浓度分别为7.81、4.79μg/ml）,差异均有统计学意义（P〈0.05）。男女流脑阳性率和浓度差异均无统计学意义（P〉0.05）。结论 2011年福建省流脑流行期健康人群带菌率低,A群流脑抗体免疫水平高于C群。A、C群流脑抗体免疫水平对健康人群有较高的免疫保护作用,但应加强3~5岁低龄儿童A群或A＋C疫苗的接种。Objective To understand the carrying rate of neisseria meningitidis（Nm）and the immune level of epidemic cerebrospinal meningitis in the healthy population in Fujian province,2011,so as to provide scientific basis for working out prevention and control measures. Methods Throat swabs and serum specimens were collected from different age groups,serum antibodies were detected by ELISA,and then susceptibility testing was detected by E-test.The data were analyzed by SPSS 17.0. Results Nm wasn＇t detected in throat swabs（0/263）;the serum total positive rate of antibodies in Nm was 87.33%（324/371）,and the antibody positive rate in Nm serogroup A and C was 78.98%（293/371）and66.04%（245/371）respectively.The antibody positive rate in of serogroup A was higher in the 1-3years age group（87.76%,43/49）than that in the 3-5years age group（73.47%,36/49）（P〈0.05）.The antibody positive rate in the serogroup C was higher in≥20years age group（74.19%,46/62）than that in the 3-5years age group（51.02%,25/49）（P〈0.05）.The antibody positive rate and concentration in the Nm serogroup A was higher in Longyan county than that in Pingnan county among monitoring points（positive rate：87.50% vs 69.01%;concentrations：7.81μg/ml vs 4.79μg/ml）（P〈0.05）.The positive rate and concentration of antibody in Nm weren＇t statistically significant among males and females（P〈0.05）. Conclusion The carrying rate is low in the healthy population during the epidemic period in Fujian province,2011,the antibody level of epidemic cerebrospinal meningitis in group A is higher than that in the group C;the antibody level of epidemic cerebrospinal meningitis in group A and C have protective immune functions in the healthy population;the groups A and A＋C vaccination should be strengthened in the 3-5years age groups.
