摘  要:目的观察百会穴(GV20)注射腺苷A1受体激动剂(CCPA)对创伤性颅脑损伤(TBI)大鼠脑水含量及炎性介质的影响。方法将30只SD大鼠按随机数字表法分为模型组、CCPA组、二甲基亚砜(DMSO)组,每组10只。采用改良Feeney法建立大鼠TBI模型;DMSO组和CCPA组于制模前30rain百会穴注射DMSO和0.1mmol/LCCPA各20儿。伤后24h评估大鼠神经功能行为学评分,并观察各组脑水含量及大脑皮质白细胞介素(IL-1、IL-10)和肿瘤坏死因子-α(TNF-α)蛋白表达的变化。结果与模型组和DMSO组比较,CCPA组神经功能评分明显升高[分:10.90±0.99比8.60±0.84、8.95±0.99,均P〈0.01],脑水含量及脑组织内IL-1β、TNF-α水平明显降低[脑水含量:(71.30±0.44)%比(73.25±0.66)%、(72.99±0.29)%,均P〈0.05;IL-1β(灰度值):29.31±3.08比63.41±2.46、51.44±7.23,TNF-α(灰度值):43.03±4.19比70.79±8.10、60.17±11.16,P〈0.05或P〈0.01],IL-10水平明显增加[灰度值:58.33±3.14比31.30±4.17、26.26±1.65,P〈0.01]。结论百会穴注射CCPA能明显减轻TBI后脑水肿的程度和神经行为缺损症状,其机制可能是通过降低脑组织中IL-1β、TNF-α和提高IL-10的表达、减轻炎症反应而起作用的。Objective To investigate the effects of injecting adenosine A1 receptor agonist (CCPA) into Baihui (GV20) acupoint on rats with traumatic craniocerebral injury (TBI). Methods Thirty Spragur-Dawley (SD) rats were randomly divided into model group, dimethyl sulfoxide (DMSO) group and CCPA group (each n = 10). The improved Feeney method was adopted to establish the rat model of TBI. Thirty minutes before model establishment, DMSO 20 μL and CCPA (0.1 mmol/L) 20 μL were injected into the GV20 acupoint in the rat in DMSO and CCPA groups, respectively. The ethology score, cerebral water content and the protein expressions of interleukin (IL-1, IL-10) and tumor necrosis factor- α (TNF- α ) in cerebral cortex were assessed after injury for 24 hours in the three groups. Results Compared with the model group and DMSO group, in CCPA group, the nerve function was elevated significantly (10.90 ± 0.99 vs. 8.60± 0.84, 8.95± 0.99, all P 〈 0.01) and the levels of water content, the protein expression of IL-1 β, TNF- α in brain tissues were significantly decreased [brain water content: (71.30 ± 0.44)% vs. (73.25±0.66)%, (72.99 ± 0.29)%; IL-1 β (gray value): 29.31 ±3.08 vs. 63.41±2.46, 51.44± 7.23, TNF- α (gray value): 43.03 ±4.19 vs. 70.79 ±8.10, 60.17 ±11.16, P 〈 0.05 or P 〈 0.01], the protein expression of IL-10 was increased obviously (gray value: 58.33±3.14 vs. 31.30±4.17, 26.26±1.65, P 〈 0.01). Conclusions Injecting CCPA into GV20 can significantly reduce the content of brain edema and alleviate nerve behavior defect symptoms in TBI rats. Its mechanism is probably related to reducing IL-1β, TNF- α expressions and enhancing the expression of IL-10 in brain tissue, thus it plays a role of decreasing inflammatory reaction.
