80例高血压性出血患者微创清除术的疗效分析

摘　　要：目的总结应用微创液化引流术治疗高血压性壳核出血予颅内血肿的疗效。方法对80例高血压性壳核出血（血肿）患者,依据CT片,根据壳核血肿的形态、大小及患者的病情,选择适宜的穿刺点、方向,行CT平面导向下的软通道——微创液化引流术,清除血肿。结果80例患者出院70例。其中,痊愈20例,显著进步30例,进步10例,无变化10例,死亡10例,病死率为13%。70例患者中失访9例,其中61例随访6个月~3年。以日常生活能力（ADL）的标准评估患者的神经功能恢复情况：ADL1级23例,占37%;ADL2级16例,占26.7%;ADL3级12例,占20.0%;ADL4级7例,占11.1%;ADL5级3例,占4.4%。结论应用微创液化引流术治疗壳核出血,最大限度地清除血肿,避免或减轻并发症,具有较大的优越性。Objective To summarize the effect of minimally invasive treatment of hypertensive liquefied drainage putamen hemorrhage intracranial hematoma.Method 80 cases of hypertensive putamen hemorrhage（hematoma） patients,according to the CT slice,according to the form putamen hematoma,size and condition of the patient,the appropriate choice of the puncture point,direction,soft channel row CT guided the plane-micro Year liquefaction drainage,remove the hematoma.Result 80 patients were discharged from hospital 70 cases.Among them,20 cases cured,30 cases of significant progress,progress in 10 cases,no change in 10 cases,10 cases of death,mortality was 13%.70 patients were lost in 9 cases,61 cases were followed up for 6 months to 3 years.With activities of daily living（ADL） of the criteria for assessing nerve function in patients with recovery：ADL1 grade 23 cases,accounting for 37%;ADL2 grade 16 cases,accounting for 26.7%;ADL3 12 cases,accounting for 20.0%;ADL4 7 cases,accounting 11.1%;ADL5 3 cases,accounting for 4.4%.Conclusion Liquefied minimally invasive drainage for treatment of putamen hemorrhage maximize remove the hematoma,avoid or reduce complications,with great advantages.
