游离皮瓣修复术治疗全手皮肤脱套伤的护理

摘　　要：目的探讨游离皮瓣修复全手脱套伤的护理。方法选取全手皮肤脱套伤患者5例(5手),实施游离双侧股前外侧皮瓣瓦合修复术,评价患者手功能恢复效果。结果经过精心的手术和护理后,5例行移植瓦合的皮瓣全部成活,无瘢痕挛缩、破溃,大腿功能无影响,功能恢复为优者1例、良3例、可1例,优良率为80%。结论术后的护理质量可直接影响手术效果,护理人员应综合观察皮瓣区血循环,避免靠皮瓣某一因素孤立判断,预防一切可能导致血管危象的因素发生,确保皮瓣成活。Objective To explore the nursing for whole- hand degloving injury patients with repair of free skin flaps. Methods Five patients with whole- hand degloving injury（ five hands） were selected,and they were treated with tiling repair of free skin flaps in front lateral of bilateral thighs. The hand function recovery was evaluated. Results After the meticulous surgery and nursing,the skin flaps of five patients with tiling repair all survived,and no scar contracture or ulceration was observed. Function of thighs was still normal. In the five patients,one patient had favorable function recovery,three patients had good function recovery and one patient had tolerable function recovery,and the excellent rate was 80%. Conclusion The postoperative nursing quality is the key influencing factor to the success of surgery. For the blood circulation in skin flaps area,we should observe comprehensively. The insolated judgment depended on single factor of skin flaps should be avoided,and all the influencing factors may lead to the occurrence of vascular crisis should be prevented in order to ensure the survival of the skin flaps.
