脊柱肿瘤术前选择性动脉栓塞的临床应用价值

摘　　要：目的探讨脊柱肿瘤术前选择性动脉栓塞（selective arterial embolization,SAE）的临床应用价值。方法 36例脊柱肿瘤患者于外科手术术前行介入栓塞治疗,对SAE血管造影表现、栓塞材料的应用、栓塞后-外科手术间隔时间、外科手术术中出血量、外科手术时间、栓塞后并发症进行回顾性分析。结果 36例患者术中造影均见明显的肿瘤染色,共栓塞血管63支。栓塞后,肿瘤染色大部分消失;外科手术术中出血量为100-2 600ml,平均出血量1 100ml,手术平均时间163min;栓塞后48h内行外科手术;栓塞后未出现严重并发症;治疗后1年内均恢复良好。结论 SAE可有效的减少脊柱肿瘤术中出血量,缩短手术时间,降低手术风险,对脊柱肿瘤的综合治疗意义重大。Objective To evaluate the clinical application value of selective artery embolism（ SAE） in the treatment of spinal tumor. Methods Selective artery embolism（ SAE） was carried out in a total of 36 patients with spinal tumor,who were planed to be treated with surgical resection. The angiographic manifestation of SAE,embolism materials,time interval between SAE and surgery,the bleeding amount of surgery,surgical time and complications after embolization were retrospectively analyzed. Oneyear follow-up after surgery was carried out. Results Totally,63 vessels of 36 patients were embolized. The 36 cases were found tumors feed arteries accluded after embolization. In this series,the blood loss ranged from 100 - 2 600 ml with average of 1 100ml; The average operation time was 163 min; Doing surgery was within 48 h after embolization,without serious complications after embolization. Conclusion The SAE was effective to reduce the blood loss during operation for spinal tumor. Then it can effectively shorten the operation time,reduce the operation risk. So,the SAE on the comprehensive treatment of spinal tumors is of great significance.
