摘 要:目的探讨磁共振脂肪抑制技术在早期强直性脊柱炎诊断中的应用价值。方法对临床确诊的87例临床诊断为强直性脊柱炎患者行MRI检查,常规轴位T_1WI、T_2WI、T_2WI加脂肪抑制序列,及斜冠状位T2*ME3D加脂肪抑制序列。结果 87例强直性脊柱炎患者中T_1WI发现信号异常者75例,T_2WI发现信号异常者69例,加脂肪抑制序列发现信号异常者82例。其中36例脂肪抑制序列表现为骶髂关节旁骨髓水肿,30例软骨下骨板粗细不均、中断、凹凸不平,16例关节面下脂肪信号堆积。结论 MRI脂肪抑制序列能够早期发现骶髂关节强直性脊柱炎骨髓水肿或是软骨板信号异常,在临床诊断及临床抗炎治疗后随访中,MRI脂肪抑制序列有很高的临床应用价值。Objective To investigate the clinical value of magnetic resonance fat suppression sequence in the diagnosis of sacroiliac joint of ankylosing spondylitis. Methods 87 patients who were confirmed ankylosing spondylitis were examined by MRI with routine axial T1WI,T2WI,fat suppression sequence and oblique coronary fat suppression sequence. Results Signal of 75 cases was abnormal in T1WI of 87 cases of ankylosing spondylitis,signal of 69 cases was found abnormal in T2WI,signal of 30 cases was abnormal in fat suppression sequence. In fat suppression sequence,36 cases of sacroiliac joints had bone marrow edema,30 cases of the subchondral had bone plate thickness uneven and interrupted,16 cases of fat signal was accumulated signal under the joint surface. Conclusion MRI fat suppression sequence could detect changes of sacroiliac joint ankylosing spondylitis earlier in bone marrow edema or abnormal cartilage board signal,and had advantage after anti-inflammatory treatment in clinical diagnosis and follow up. Fat suppression sequence had high clinical application value.
