摘 要:目的本研究拟通过建立功能性单心室(FSV)动物模型,探讨超声新技术评估FSV收缩功能的可行性。方法对12只猪建立左室型单心室(SLV)的动物模型,分别用实时三维超声(RT-3DE)、组织多普勒(TDI)和速度向量成像(VVI)测定模型基础状态下、Glenn模型、Fontan模型和改变心肌收缩力状态下的心功能。结果 1)Glenn和Fontan模型左室射血分数(LVEF)均较基础状态显著下降,整体长轴应变(GLS)和整体环向应变(GCS)显著下降,扭转和扭力显著下降;左心室游离壁和室间隔处Tei指数显著升高;左室游离壁各节段的纵向速度、纵向位移、纵向应变显著减少;2)增加心肌收缩力状态的LVEF、GLS和GCS均显著增加,左心室扭转和扭力明显增加;左心室游离壁和室间隔处测定的Tei指数均明显降低。左室游离壁基底段的纵向速度、位移、应变和应变率均显著增加。结论 3DE、TDI和VVI可以反映功能性单心室腔肺分流术后模型的心功能。Objective To creat the animal model of FSV( functional single ventricle) and to evaluate the feasibility of the new echocardiographic techniques in assessing the systolic function of FSV. Methods Animal models of single left ventricle( SLV) were created in 12 piglets. Ventricular function was determined by real time three-dimensional echocardiography( RT-3DE),velocity vector imaging( VVI) and tissue Doppler imaging( TDI) in the animal models of the baseline,Glenn circulation,Fontan circulation and acute changes in contractile state. Results 1) Both Glenn and Fontan model resulted in lower left ventricular ejection fraction( LVEF),global longitudinal strain( GLS),global circumferential strain( GCS),twist and torsion than those obtained at baseline. Tei indexes markedly increased at both LV free wall and intraventricular septum. Longitidunal velocity,longitidunal displacement and longitidunal strain were significantly lower at various segments of LV free wall; 2) Animal models of increased myocardial contractility resulted in improved LVEF,GLS,GCS,twist and torsion. Tei indexes markedly decreased at both LV free wall and intraventricular septum. Longitidunal velocity,longitidunal displacement,longitidunal strain and longitidunal strain rate were significantly higher at the basal segments of LV free wall. Conclusion It is feasible to assess the ventricular function of FSV using VVI,TDI and RT-3DE.
