摘 要:目的探究Bi PAP无创呼吸机治疗慢性阻塞性肺疾病(慢阻肺)对患者细胞间粘附分子-1、E-选择素和D-二聚体浓度的影响。方法将慢阻肺患者80例随机分为两组,对照组患者40例接受常规治疗,慢阻肺组患者40例在常规治疗基础上联用Bi PAP无创呼吸机治疗。采用酶联免疫吸附法检测慢阻肺患者治疗前后细胞间粘附分子-1、E-选择素及D-二聚体的浓度并比较,分析Bi PAP无创呼吸机治疗对以上指标的影响。结果治疗前慢阻肺组的ICAM-1、E-选择素、D-二聚体水平与对照组相比无显著的统计学意义(P>0.05);治疗后两组的ICAM-1、E-选择素、D-二聚体水平相比治疗前均有下降,慢阻肺组治疗前后差异具有显著性(P<0.05)并显著低于对照组(P<0.05)。结论 Bi PAP无创呼吸机治疗慢阻肺有一定临床疗效,能显著降低细胞间粘附分子-1、E-选择素、D-二聚体的浓度并减轻患者炎症反应。Objective To investigate the impact of BiPAP noninvasive ventilator for patients with chronic obstructive pulmonary disease(COPD) on the level of intercellular adhesion molecule-1 (ICAM-1), E-selection and Ddimer concentration. Methods 80 patients with COPD were randomly divided into two groups. 40 patients received conventional treatment as the control group, and 40 patients received BiPAP noninvasive ventilator basing on conventional treatment as the COPD group. Then the level of ICAM-1, E-selection and D-dimer were detected by enzymelinked immunosorbent assay (ELISA). Results The levels of ICAM-1, E-selection and D-dimer showed no signifi- cant difference before the treatment between the two groups ( P 〉 0. 05 ). After 1 month of treatment with BiPAP noninvasive ventilator, the levels of three indicators above decreased more significantly in the COPD group (P 〈 0. 05), and they were significantly lower than those in the control group (P 〈 0.05 ). Conclusion There is a certain effect of BiPAP noninvasive ventilator for patients with COPD, which can decrease the level of ICAM-1, E-selection and D-dimer concentration and improve the inflammation reaction significantly.
