摘  要:目的探讨新生儿胆红素脑病磁共振成像(MRI)及磁共振波谱(MRS)的特征。方法选择2014年1月至2015年1月平邑县中医医院新生儿科诊治的胆红素脑病患儿30例为观察组,另选择同时期30例健康新生儿为对照组。观察胆红素脑病患儿MRI平扫特征,测量并比较两组苍白球的T1加权像(T1WI)及T2加权像(T2WI)信号强度。于MRS图像中测量并比较N-乙酞天冬氨酸/肌酸复合物(NAA/Cr)、胆碱复合物/肌酸复合物(Cho/Cr)、乳酸/肌酸复合物(Lac/Cr)、谷氨酸及谷氨酞胺复合物/肌酸复合物(Glx/Cr)和肌醇/肌酸复合物(m I/Cr)的差异。结果观察组左侧和右侧苍白球T1WI信号强度均高于对照组(890±136比676±52,885±132比683±76),差异有统计学意义(P<0.01);两组双侧苍白球T2WI信号强度比较差异无统计学意义(P>0.05)。两组NAA/Cr、Cho/Cr、Lac/Cr比较差异无统计学意义(P>0.05),但观察组Glx/Cr和m I/Cr显著高于对照组(0.51±0.13比0.32±0.11,0.73±0.20比0.55±0.16),差异有统计学意义(P<0.01)。结论胆红素脑病患儿常规MRI表现为双侧苍白球对称性T1WI高信号,MRS则显示NAA/Cr、Cho/Cr、Lac/Cr正常,Glx/Cr和m I/Cr水平升高。Objective To investigate the clinical features of magnetic resonance imaging( MRI) and magnetic resonance spectroscopy( MRS) of neonatal bilirubin encephalopathy. Methods Total of 30 cases of neonatal bilirubin encephalopathy admitted to Pingyi County Chinese Medicine Hospital from Jan. 2014 to Jan. 2015 were included as an observation group,and 30 healthy newborns during the same period were included as a control group. The characteristics of MRI scan in children with neonatal bilirubin encephalopathy were observed,the intensity of T1 WI and T2 WI signal of globus pallidus of the two groups were measured and compared. The differences of m I / Cr,Lac / Cr,Cho / Cr,Glx / Cr and NAA / Cr were measured and compared in MRS images. Results Left and right globus pallidus T1 WI signal intensity of the observation group were higher than the control group( 890 ± 136 vs 676 ± 52,885 ± 132 vs 683 ± 76),the differences were statistically significant( P〈0. 01); bilateral globus pallidus T2 WI signal intensity difference was not statistically significant( P〉0. 05). NAA / Cr,Cho / Cr,Lac / Cr of the two groups had no significant difference( P〈0. 05),but Glx / Cr and m I / Cr of the observation group were significantly higher than the control group( 0. 51 ± 0. 13 vs 0. 32 ± 0. 11,0. 73 ± 0. 20 vs 0. 55 ± 0. 16),the differences were statistically significant( P〈0. 01). Conclusion The feature of the conventional MRI in the patients with neonatal bilirubin encephalopathy is high and symmetrical bilateral globus pallidus T1 WI,and MRS features are normal levels of NAA / Cr,Cho / Cr,Lac / Cr and elevated levels of Glx / Cr and m I / Cr.
