低位产钳术和剖宫产对产妇和婴儿预后结局的对比分析

摘　　要：目的对比分析低位产钳术和剖宫产对产妇和婴儿预后结局的影响。方法回顾性分析2012年3月至2015年3月上海市嘉定妇幼保健院收治的260例产妇的临床资料,依据分娩方式不同分为低位产钳组和剖宫产组,每组130例。比较两组产妇及婴儿预后结局。结果低位产钳组与剖宫产组产妇并发症发生率分别为15.4%(20/130)、16.2%(21/130),差异无统计学意义(P>0.05);两组婴儿体质量[(3.8±0.7)kg比(3.7±0.7)kg]、5 min Apgar评分[(9.2±0.7)分比(9.3±0.9)分]比较差异无统计学意义(P>0.05)。低位产钳组胎头娩出时间、产后出血量、住院时间均显著低于剖宫产组[(6.1±1.9)min比(7.9±1.5)min,(139±22)m L比(228±35)m L,(4.2±1.3)d比(6.9±1.9)d],差异有统计学意义(P<0.01)。结论低位产钳术可缩短产妇分娩时间、减少出血量、降低产妇和胎儿的并发症发生率,值得临床推广应用。Objective To compare the influence of low forceps and Cesarean delivery on maternal and infant outcomes. Methods Clinical information of 260 parturients treated in hanghai Jiading Maternal and Child Health Hospital from Mar. 2012 to Mar. 2015 were retrospectively analyzed. All the parturients were divided into low forceps group and Cesarean section group,130 cases each. The maternal and infant outcomes of the two groups were compared. Results The incidence of complications of the low forceps group and Cesarean section group were 15. 4%（ 20 /130）,16. 2%（ 21 /130）,the difference had no statistically significance（ P〉0. 05）. The infant body mass[（ 3. 8 ± 0. 7） kg vs（ 3. 7 ± 0. 7） kg],5 min Apgar score[（ 9. 2 ±0. 7） scores vs（ 9. 3 ± 0. 9） scores]of the two groups had no statistical difference（ P〉0. 05）. The fetal head delivery time,postpartum bleeding,hospitalization time of the low forceps group were significantly lower than the Cesarean section group[（ 6. 1 ± 1. 9） min vs（ 7. 9 ± 1. 5） min,（ 139 ± 22） m L vs（ 228 ± 35） m L,（ 4. 2 ± 1. 3） d vs（ 6. 9 ± 1. 9） d,P〈0. 01]. Conclusion Low forceps delivery can shorten delivery time,reduce the amount of bleeding and reduce the incidence of maternal and fetal complications. It is worthy of clinical application.
