多层螺旋CT双期血管成像技术在下肢深静脉血栓形成中的诊断价值研究

摘　　要：目的:探讨多层螺旋CT双期血管成像技术(MSCTA)在下肢深静脉血栓形成(DVT)中的诊断价值.方法:对收治的46例疑诊为DVT的患者行MSCTA检查,其中32例同时行数字减影血管造影(DSA)检查.结果:MSCTA检查46例患者中有3例患者正常,43例患者存在下肢深静脉血栓形成,左侧者27例,右侧者14例,2例为双侧.行32例DSA检查的患者检查结果与MSCTA一致,诊断符合率为100%.MSCTA发现股深静脉2段,而DSA检查未能显示,DSA检查发现3例有下肢深静脉瓣膜功能不全,MSCTA检查未能发现.有44例患者至少存在一侧股总静脉正常显影,1期静脉显影结果 1级11例,2级28例,3级4例,4级1例,2期显影结果 1级12例,2级28例,3级3例,4级1例,显影效果双期比较,差异无统计学意义(P>0.05);股总静脉正常的患者的平均CT值及标准差,1期扫描为(101.48±12.94)、(13.59±2.16),2期扫描为(89.29±10.47)、(9.14±1.98),双期比较,差异具有统计学意义(P<0.05).结论:MSCTA用于下肢深静脉血栓形成的诊断,与DSA比较具有较高的诊断符合率,并且具有检查无创伤性,具有较高的临床价值.AIM： To assess the clinical value of dual-phase multi-slice spiral CT angiography in deep venous thrombosis of lower extremity. METHODS： A total of 46 patients with suspective diagnosed as DVT were treated by multi-slice spiral CT angiography（ MSCTA）,including 32 cases at the same time treated by digital subraction angiography（ DSA）. RESULTS： In 46 patients treated by MSCTA inspection,3 patients were normal,43 patients existed lower extremity deep vein thrombosis,among them 27 cases on the left side,14 cases on the rightside,and2 cases on both sides. 32 cases used DSA examination had the same results with MSCTA examination,and the diagnostic coincidence rate was 100%. MSCTA found 2 sections of strands of deep vein,while DSA examination failed to display and DSA examination showed 3 cases had lower extremity deep vein valve insufficiency,while MSCTA inspection failed to find. There were 44 patients with at least one side strands of total intravenous normal development,according to 1 phase vein development results,Grade 1 in 11 cases,Grade 2 in 28 cases Grade 3 in 4,Grade 4in 1,while according to 2 phase development results,Grade 1 in12 cases,Grade 2 in 28 cases,Grade 3 in 3,Grade 4 in 1. Comparing both results,there was no statistically significant difference（ P0.05）; Stock total intravenous normal patients with a mean CT value and standard deviation,1 phase scanning results were101.48±12.94,13.59±2.16,2 phase scanning results were 89.29±10. 47,9. 14 ± 1. 98,showing statistically significant difference（ P0.05）. CONCLUSION： MSCTA used in the diagnosis of DVT,and compared with DSA has high diagnostic coincidence rate and no traumatic examination,which has high clinical value.
