摘 要:目的研究幽门螺杆菌(Hp)感染对患者机体外周血中淋巴细胞亚群的影响,探讨流式细胞术在Hp感染检测中的作用。方法 2013年10月-2014年10月采用13 C呼气试验筛选Hp感染人群,利用流式细胞术检测50例胃炎HP阳性患者、33例胃炎HP阴性患者,51名体检健康Hp阳性和73名正常人外周血中CD3+、CD_4^+、CD8+、CD_4^+/CD_8^+、CD_(19)^+和CD_(16)^+/_(56)^+的淋巴细胞亚群比例,数据采用SPSS 16.0软件进行统计分析。结果胃炎组Hp阳性和阴性患者中单核细胞相对值分别为(5.95±0.80)%和(6.27±1.77)%,与健康组Hp阴性者(2.05±0.73)%相比,差异有统计学意义(P<0.05);与健康组Hp阴性者相比,胃炎组Hp阳性患者CD_4^+/CD_8^+升高,CD_(19)^+比率下降,差异有统计学差异(P<0.01);健康组Hp阳性者CD_4^+/CD_8^+与Hp阴性者相比结果升高,差异有统计学意义(P<0.05);胃炎组Hp阳性患者CD_3^+比例较Hp阴性患者升高,差异有统计学意义(P<0.05),其他各组间不同指标比例差异无统计学意义。结论在Hp感染至胃炎发生的过程中,外周血中淋巴细胞亚群表现出一定的动态变化,利用流式细胞术检测外周血中淋巴细胞亚群比例对监测Hp感染具有重要作用。OBJECTIVE To discuss the relationship between lymphocyte subsets in the peripheral blood and Helicobacter pylori(Hp)infections,and to investigate the role of flow cytometry in the detection of Hp infection.METHODS From Oct.2013-Oct.2014,^13 C breath test was used to detect patients with HP infection,flow cytometry was used to determine the percentage of CD3^+,CD4-^+,CD8^+,CD4^+/CD8^+,CD19^+ and CD16^+/56^+in peripheral blood of 50 Hp+ patients with chronic gastritis,33 Hp- patients with chronic gastritis,51 Hp+healthy people and 73 healthy people.Data were analyzed with SPSS 16.0.RESULTS The relative values of monocytes of gastritis Hp+ group and Hp- group were respectively(5.95±0.80)%and(6.27±1.77)%.Compared with the relative value of mononuclear cells(2.05±0.73)% of healthy Hp- group,the difference was significant(P〈0.05).Compared with healthy Hp- group,the ration of CD4^+/CD8^+in gastritis Hp+ group increased,but the rate of CD_(19)-+ decreased,difference was significant(P〈0.01).The ratio of CD4^+/CD8^+in healthy Hp+group increased,compared with Hp- group,difference was significant(P〈0.05).Compared with gastritis Hp- group,the percentage of CD3+in the gastritis Hp+ increased,difference was significant(P〈0.05).The differences of percentages between other groups were not significant.CONCLUSIONIn the progression of gastritis induced by H.pylori infection,lymphocyte subsets in peripheral blood showed a dynamic change.The use of flow cytometry to detect the percentage of lymphocyte subsets in peripheral blood has positive effect on monitoring H.pylori infection.
