神经内科感染患者降钙素原水平与白细胞比值变化分析

摘　　要：目的研究神经内科感染患者降钙素原(PCT)水平与中性粒细胞/淋巴细胞比值(NLR)变化,为提高诊断率提供依据。方法选取2014年12月-2015年12月医院诊治的38例神经内科感染患者资料进行分析,将其设为试验组,选取同期入院的38例神经内科未感染患者设为对照组,采用电化学发光法检测PCT水平,采用乳胶比浊法测定C-反应蛋白(CRP)水平,采用希思美康XS800i全自动五分类血细胞分析仪进行NLR测定,分析神经内科感染患者PCT水平与NLR变化。结果试验组38例感染患者中共分离出病原菌76株,其中革兰阴性菌32株占42.11%,革兰阳性球菌10株占13.16%,真菌34株占44.73%;试验组患者PCT、CRP水平以及NLR,显著高于对照组患者,差异有统计学意义(P<0.05);试验组患者中性粒细胞水平显著低于对照组患者,差异有统计学意义(P<0.05)。结论神经内科感染患者PCT水平与NLR变化较大,能动态评估患者病情严重程度,值得推广应用。OBJECTIVE To study the changes of procalcitonin（PCT）and neutrophil lymphocyte ratio（NLR）in patients with infections of department of internal neurology,so as to provide the basis for improving diagnosis.METHODS A total of 38 infected patients of department of internal neurology from Dec.2014 to Dec.2015 were collected and analyzed as test group,and another 38 cases of hospitalized uninfected patients of department of internal neurology at the same period were collected as control group.The PCT level was detected by electric chemiluminescence,CRP level was measured using latex turbidimetry,neutrophil lymphocyte ratio was determined using Heath Meikang XS800 iautomatic hematology analyzer,and the changes of PCT level and neutrophil lymphocyte ratio in patients were analyzed.RESULTS A total of 76 strains of pathogens were isolated from 38 cases of infected patients of test group,including 32 strains of gram-negative bacteria,accounting for 42.11%,10 strains of gram-positive bacteria,accounting for 13.16%,and 34 strains of fungi,accounting for 44.73%,respectively.The levels of PCT,CRP and NLR of test group were significantly higher than those of control group（P〈0.05）.The neutrophils level of test group was significantly lower than that of control group（P〈0.05）.CONCLUSION The changes of PCT level and NLR in the patients with infection of department of internal neurology are great,and they can be used for dynamical assessment of patients＇ conditions,so it should be widely applied.
