增强CT与超声对颈动脉粥样硬化斑块评价的对照研究

摘　　要：目的对比研究增强CT与超声对颈动脉粥样硬化斑块的检测,确定优势方法。方法对170例严格纳入标准后的患者进行强化CT与超声检查,以数字法随机分成观察组和对照组,各85例。观察组患者进行超声检测;对照组患者则进行增强CT检查。结果每组有85例患者,分别有680节段。观察组检测出斑块总数为259,占38.09%(259/680),检测率显著高于对照组的32.65%(222/680),差异有统计学意义(P<0.05)。采用超声检测的观察组发现290节颈动脉斑块,检测率为42.65%(290/680),采用增强CT的对照组发现250节颈动脉斑块,检测率为36.76%(250/680),观察组检测率显著高于对照组检测率,差异有统计学意义(P<0.05)。观察组检测出的狭窄节段共299节,有381节正常,检测率为43.97%(299/680),显著高于对照组的检测率38.38%(261/680),差异有统计学意义(P<0.05)。结论综上所述,超声检测颈动脉粥样硬化斑块的准确性高,误差低,优势显著,能很好的确定斑块成分和分布,简便无创,值得广泛推广。Objective To contrast enhanced CT and ultrasound evaluation of carotid atherosclerotic plaques. Methods 170 patients of the hospital and enhanced CT and ultrasound. In digital method randomly divided into observation （85 cases） and control （85 cases）. Observation group of patients in ultrasonic testing. The control group patients were enhanced CT examination.Results 85 cases of patients in each group, there were 680 segments. Observation group to detect the plaques for a total of 259, accounting for 38.09% （259/680）, signiifcantly higher detection rate of 32.65% （222/680）, the difference was statistically signiifcant （P〈0.05）. Using ultrasonic testing of the observation group that section 290 carotid plaques, the detection rate was 42.65% （290/680）, the enhanced CT group found that section 250 carotid plaques, the detection rate was 36.76% （250/680）, the observation group had a signiifcantly higher detection rate detection rate. The difference was statistically signiifcant （P〈0.05）. Observation group to detect the stenosis segment section 299, section 381 is normal, the detection rate was 43.97% （299/680）, signiifcantly higher detection rate was 38.38% （261/680）. The difference was statistically signiifcant （P〈0.05）.Conclusion To sum up, ultrasonic detection of carotid atherosclerotic plaques high accuracy, low error, advantage, can be a very good determine plaque composition and distribution, simple and noninvasive, is worth popularizing widely.
