依帕司他治疗糖尿病神经病变下肢溃疡的效果

摘　　要：目的评估依帕司他改善糖尿病神经病变下肢溃疡的效果。方法选择陕西省安康市中心医院2011年7月~2015年6月收治的2型糖尿病神经病变合并下肢溃疡患者（Wagner分级1~2级）62例,将其随机分为治疗组（32例）和对照组（30例）,对照组仅常规控制血糖,治疗组在控制血糖的同时给予依帕司他口服,疗程12周。分析两组患者治疗后溃疡愈合率、溃疡缩小百分比以及溃疡缩小50%所需的时间。结果治疗12周后,治疗组溃疡愈合率为40.6%,对照组为33.3%,差异无统计学意义（P=0.46）;治疗组溃疡缩小60%的患者占47.4%,高于对照组（15.0%）,差异有统计学意义（P=0.04）;治疗组溃疡缩小50%所需的中位时间为33.5 d,明显短于对照组（53.0 d）,差异有统计学意义（P=0.04）。结论依帕司他能够缩短糖尿病神经病变下肢溃疡的愈合时间。Objective To evaluate the effect of Epalrestat in improving lower limb ulcer in patients with diabetic neuropathy. Methods Sixty two cases of patients with diabetic neuropathy and lower limb ulcer（grade 1-2 according to the Wagner classification） admitted to Center Hospital of Ankang City from July 2011 to June 2015 were selected and randomly divided into treatment group（32 cases） and control group（30 cases）. The control group was treated with regular blood glucose control only, based on regular blood glucose control, the treatment group was treated with Epalrestat orally for 12 weeks. The ulcer healing rate, the ulcer lessening percentage and the time of ulcer lessening for 50% were analyzed. Results After treatment for 12 weeks, the ulcer healing rate of treatment group was 40.6%, which of the control group was 33.3%, there was no statistically significant difference between the two groups（P = 0.46）. The percentage of ulcer lessening for 60% in the treatment group（47.4%） was significantly higher than that of control group（15.0%）, the difference was statistically significant（P = 0.04）. The time of ulcer lessening for 50% in the treatment group was 33.5 days, which was shorter than that of control group（53.0 days）, the difference was statistically significant（P = 0.04）. Conclusion Epalrestat can shorten the healing time of lower limb ulcer in patients with diabetic neuropathy.
