摘　　要：目的研究胆道疾病儿童肝组织中热休克蛋白72（HSP72）与肝脏纤维化关系及其表达的意义。方法收集2013年6月至2015年6月深圳市儿童医院小儿肝胆外科行手术的胆总管囊肿（A组）和胆道闭锁（B组）患儿各30例的临床和病理资料,采用SP免疫组化法测定肝脏组织中HSP72的表达。结果肝纤维化Ohkuma′s分级B组高于A组（Z=7.04,P〈0.01）;HSP72染色阳性率B组高于A组（χ^2=41.71,P〈0.01）;HSp72表达强度B组高于A组（Z=6.31,P〈0.01）;B组中HSP72在肝纤维化程度较重组的表达低于较轻组（Z=-4.16,P〈0.01）,B组中HSP72表达强度与手术时年龄呈负相关（r=-0.704,P〈0.01）,与术后胆管炎发作呈负相关（r=-0.371,P〈0.05）,B组中HSP72表达强度较高者其自体肝生存率较高（χ2=9.482,P〈0.01）。结论胆道疾病患儿肝脏组织中HSP72的表达与肝组织纤维化程度密切相关,HSP72表达强度可能为胆道闭锁患儿预后的影响因素之一,可作为评价预后的参考指标。Objective：To study the relationship between heat shock protein 72（HSP72）and liver fibrosis in children with biliary tract disease and the significance of its expression. Methods：the clinical and pathological data of 60 patients with choledochocyst（A group,30 cases）and biliary atresia（B group,30 cases）were collected from June 2013 to June 2015,and the expression of HSP72 in liver tissue was determined by SP immunohistochemical method. Results：group B was higher than group A in liver fibrosis Ohkuma′s grade（Z=7.04,P〈0.01）;group B was higher than group A in HSP 72 staining positive rate（χ2=41.71,P〈0.01）;group B was higher than group A in the expression of HSp72 intensity（Z=6.31,P〈0.01）;the greater group of liver fibrosis degree with biliary atresia was less than the lighter group in the expression of HSP72（Z=-4.16,P〈0.01）. In group B,HSP72 expression intensity and operation age showed negative correlation（r=-0.704,P〈0.01）,HSP72 expression intensity and postoperative cholangitis showed a negative correlation（r=-0.371,P〈0.05）. In group B,when the expression of HSP72 was higher,the autologous liver survival rate was higher also（χ2=9.482,P〈0.01）. Conclusion：The expression of HSP72 in liver tissue of patients with biliary tract disease is closely related to the degree of liver tissue fibrosis. HSP72 expression may be one of the factors affecting the prognosis of patients with biliary atresia,and can be used as a reference index for evaluating prognosis.
