摘 要:目的 探讨肺炎支原体肺炎(MPP)患儿外周血T辅助细胞亚群水平变化及其与喘息发生的关系.方法 选择2014年12月至2015年10月我院收治的肺炎患儿307例,其中134例MPP患儿设为MPP组,173例非MPP患儿设为非MPP组,收集同期140例于我院体检的健康儿童血清设为健康对照组,采用双抗体夹心酶联免疫吸附测定(ELISA)方法检测三组血清中γ干扰素(IFN-γ)、IL-4等细胞因子水平.结果 MPP组患儿血清IFN-γ、IFN-γ/IL-4低于非MPP组,IL-4水平高于非MPP组,MPP组血清IFN-γ、IL-4水平较健康对照组高,而IFN-γ/IL-4较健康对照组低,差异有统计学意义(P<0.05);非MPP组血清IFN-γ、IL-4水平较健康对照组高,差异有统计学意义(P<0.05),两组IFN-γ/IL-4差异无统计学意义(P >0.05);134例MPP患儿分为喘息组和非喘息组,喘息组患儿血清IL-4水平高于非喘息组,差异有统计学意义(P<0.05),两组IFN-γ、IFN-γ/IL-4差异无统计学意义(P>0.05).结论 Th1/Th2细胞因子IFN-γ及IL-4的释放,Th2细胞介导的免疫占优势,Th1/Th2比例失衡,参与了MPP的发病过程.而IL-4的升高在MPP患儿喘息的发生、发展中起重要作用.Objective To explore the peripheral blood T cell subsets level changes in children with mycoplasma pneumoniae pneumonia (MPP) and its relationship with breathing.Methods 307 cases of patients with pneumonia in our hospital from December 2014 to October 2015 were selected,134 cases of children with mycoplasma pneumoniae pneumonia as MPP group,173 cases of children with non mycoplasma pneumoniae pneumonia as NMPP group,and 140 cases of healthy volunteers checked up in our hospital during the same period were selected as healthy control group.The IFN-γ and IL-4 in serum of three groups were measured by interferon-γ enzyme linked immunosorbent assay (ELISA).Results The level of IFN-γ,IFN-γ/IL-4 in MPP group were lower than that of NMPP group.The level of IL-4 in MPP group was higher than that of NMPP group.The level of interferon-γ(IFN-γ) and IL-4 in MPP group were higher than healthy control group,but IFN-γ/IL-4 was lower than healthy control group.The differences had statistic significance (P 〈0.05).The level of IFN-γ and IL-4 in NMPP group were higher than healthy control group,the differences had statistic significance (P 〈0.05).The difference of IFN-γ/IL-4 between NMPP group and healthy control group didn't show significant difference (P 〉0.05).134 cases of children with mycoplasma pneumoniae pneumonia were divided into breathing group and unbreathing group.The level of IL-4 in breathing group was higher than un-breathing group' s,the differences was significant (P 〈 0.05).The differences of IFN-γ and IFN-γ/IL-4 between breathing group and un-breathing group did not show significant difference (P 〉0.05).Conclusions IFN-γ and IL-4 released in Th1/Th2,immune mediated by Th2 cell,ratio imbalance of Th1/Th2,those reasons participated the pathogenesis of mycoplasma pneumoniae pneumonia.But the increased level of IL-4 plays an important role during the occurrence and development of breathing of children with mycoplasma pneumoniae pneumonia.
