海南省2012-2014年108例孕妇生殖道支原体感染及药敏试验研究

摘　　要：目的了解海南省医院产科住院孕妇生殖道支原体培养结果构成情况及其对抗生素药物的敏感性,分析支原体耐药性的变化趋势,为临床上合理使用抗生素提供参考依据。方法收集2012-2014年海南省两家三甲医院产科病房住院孕妇生殖道支原体感染及常用9种抗生素药敏结果,采用Epidata软件进行资料建库,用SPSS19.0软件对资料进行分析。主要分析生殖道感染孕妇的基本情况、生殖道支原体感染及抗生素药物敏感性情况,并分析支原体耐药性的变化趋势。结果共检测108例住院孕妇生殖道支原体感染情况,其中解脲脲原体感染例数最多,达102例,构成比高达94.44%;解脲脲原体对氧氟沙星、罗红霉素、阿奇霉素耐药率较高,分别为28.43%(29/102)、21.57%(22/102)、17.65%(18/102);2012-2014年多重耐药率分别为13.04%(3/23)、35.56%(16/45)、26.47%(9/34),合计多重耐药率为27.45%(28/102)。对克拉霉素的耐药有上升趋势,对交沙霉素的耐药有下降趋势。结论海南住院孕妇生殖道支原体感染中以解脲脲原体感染为主,其对常见抗生素敏感不一,临床上应行支原体培养并药敏试验,合理使用抗生素。Objective To understand the prevalence of genital mycoplasma infection in pregnant women and mycoplasma drug susceptibility,and to provide reference for clinical practice. Methods A retrospective analysis was conducted on the results of mycoplasma infections in genital tract of hospitalized pregnant women and on 9drug susceptibility tests from 2012 to 2014.SPSS 19.0software was used to analyze the data. Results Out of 108 cases with positive mycoplasma,102 cases were infected with Ureaplasma urealyticum（Uu）.The resistance rates of Uuto ofloxacin,luoerythromycin and azithromycin were among the highest,accounting for 28.43%,21.57% and 17.65%,respectively.The multiple resistance rates were 13.04%,35.56% and 26.47%in 2012,2013 and 2014.The resistance to clarithromycin increased while to josamycin decreased. Conclusions The predominant genital mycoplasma infection of pregnant women in Hainan was Uu and the resistance rate varies among commonly used antibiotics.Therefore,it is suggested that drug susceptibility test should be performed to guide the antibiotics use.
