支气管动脉128层螺旋CT造影分析

摘　　要：目的探讨128层螺旋CT血管造影对支气管动脉（BA）的检测能力及BA异常表现。方法对62例咯血患者同时行支气管动脉CTA和DSA检查,对BA进行分析,比较两者显示率及异常表现。结果 CTA共检出BA 162支,DSA共检出BA 182支,包含CTA所检出的BA;CTA与DSA同时检出BA 162支,一致率89.01%;CTA未检出BA 20支,管径为（0.82±0.13）mm（0.50~1.00 mm）;CTA检出增粗的BA 138支,表现为主干不同程度增粗、纡曲,伴或不伴有中途分支增多、增粗,末梢紊乱血管网形成。CTA检出支气管动脉-肺动脉瘘19例,DSA检出支气管动脉-肺动脉瘘58例。结论 128层螺旋CT血管造影能全面显示主要BA,当BA直径≤1 mm时,其显示能力明显不及DSA;CTA能检出支气管动脉-肺动脉瘘。Objective To investigate the application of 128-slice CT angiography（CTA） in the detection and fi ndings of bronchial arteries. Methods Sixty-two patients with hemoptysis underwent bronchial arterial CTA and DSA. In all detected bronchial arteries, a comparative analysis was performed between the CTA and DSA images. Results CTA detected 162 bronchial arteries, DSA detected 182 bronchial arteries. A total of 162 bronchial arteries were both detected by CTA and DSA with a coincidence ratio of 89.01%. CTA failed to detect 20 bronchial arteries, with diameter of（0.82±0.13）mm（0.50-1.00 mm）. CTA detected 138 dilated bronchial arteries, performance as the trunk of varying degrees of dilation, tortuosity, with or without half-way branch increased, dilated and disordered vascular network formation. CTA detected 19 cases of bronchial artery to pulmonary circulation shunt（BPS） while DSA detected 58 cases. Conclusions 128-slice spiral CTA of bronchial artery can display the main bronchial arteries entirely and detect BPS, but its display ability is greatly inferior to that of DSA when diameter of bronchial artery is less than 1 mm.
