摘  要:卵巢功能早衰(POF)是指女性在40岁以前发生自然绝经,发病率呈逐年升高的趋势,且有低龄化倾向,可导致不孕不育,雌激素水平低下,对骨骼、心血管系统等也会造成一定影响。POF病因复杂、机制不明,除了部分遗传因素、自身免疫性因素、感染因素、酶缺陷及医源性因素等可明确病因以外,大部分无明确病因可循(即特发性POF),是临床治疗的一个难题。近期研究表明,自身免疫是POF的重要发病机制之一。自身抗体的产生及细胞免疫功能异常均参与POF的发生。目前的治疗方法疗效均不理想,阐明POF的分子免疫学发病机制及与神经内分泌间的内在联系,将是寻求预防、治疗POF有效方法的关键。The premature ovarian failure(POF) generally describes a syndrome of gonadal failure before the age of 40.In recent years, the incidence of POF in fertile woman has increased year by year and there is the development trend of younger age. POF, a serious life-changing condition that affects young women. Low estrogen levels also affect cardiovascular system and skeletal system, etc. Infertility is a common consequence of this entity. The known cause of this condition includes: genetic aberrations, autoimmune ovarian damage, infection, enzyme defect and iatrogenic factors, although in majority of cases the underlying cause is not been identified. A growing body of research suggests that autoimmunity is one of the important pathogenesis of POF. Autoantibody and abnormal cellular immunity function are thought to play an important role in the pathogenesis of POF. Currently, there are no ideal treatment options for POF. To elucidate the molecular immunology mechanism of POF and its relationship with neuroendocrine function will be the key to seek effective prevention and treatment.
