品管圈联合个案追踪法对乳腺癌术后患者患肢功能锻炼依从性及康复的影响

摘　　要：目的探讨品管圈联合个案追踪法对乳腺癌术后患者患肢功能锻炼依从性及康复的效果。方法选择80例乳腺癌改良根治术患者，按时间段分为实验组和对照组各40例，对照组采用常规术前术后护理、健康教育和使用乳腺癌术后康复操作为功能锻炼方法，实验组在此基础上开展品管圈活动，引人PDCA循环管理，联合个案追踪法，术后电话随访进行跟踪式康复指导3个月。出院前和出院后3个月评价两组患者患肢功能锻炼依从性以及康复效果。结果实验组患肢功能锻炼依从性得分明显高于对照组，差异有统计学意义（P〈0．05）；实验组出院前后总依从性得分差别不明显，差异无统计学意义（P〉0．05）；而对照组出院后3个月得分较出院前低，差异有统计学意义（P〈0．05）；实验组患肢肩关节的外展、前屈、后伸、旋内旋外的角度明显优于对照组，差异有统计学意义（P〈0．05）。结论乳腺癌术后患者开展品管圈活动，引入PDCA循环管理，联合个案追踪法，电话随访延续性护理干预，能有效提高乳腺癌术后患者患肢功能锻炼依从性，促进患肢功能康复，改善患者生活质量，使患者早日回归社会。Objective To explore the effect of quality control circle combined with case tracking method on the limb functional exercise compliance and rehabilitation after breast cancer operation. Methods 80 breast cancer patients who accepted modified radical mastectomy were divided into experimental group （n=40） and control group （n=40）. Patients in control group accepted routine nursing and health education, and patients in experimental group accepted quality control circle, PDCA cycle management, case tracking method, and rehabilitation guide for three months through telephone follow-up after operation. The compliance and rehabilitation effect were evaluated at the end of three months after being discharged. Results The score of functional exercise compliance in experimental group was significantly higher than that in control group, with statistically significant difference （P〈0.05）. The angles of abduction, flexion, extension, and internal-external rotation of shoulder joint in experimental group were obviously better than those in control group （P〈0.05）. Conclusions Quality control circle combined with case tracking method, PDCA cycle management, and telephone follow-up can improve the compliance of limb functional exercise after breast cancer operation, promote the recovery of limb function, improve the life quality, and urge patients to return to the society as soon as possible.
