老年顽固性鼻出血不同手术方法治疗效果分析

摘　　要：目的：研究不同手术方法治疗老年顽固性鼻出血的临床疗效。方法随机将2012年7月至2014年11月68例老年难治性鼻出血患者分为观察组与对照组，每组34例，观察组采取鼻镜下电凝或手术治疗，对照组行常规前后鼻填塞，比较两组患者的临床疗效、不良反应及再出血率。结果观察组治疗总有效率为97.06％，与对照组的67.65％比较差异有统计学意义（P ＜0.05）；观察组手术时间为（19.85±3.06）min，与对照组的（10.36±2.69）min 比较差异有统计学意义（P ＜0.05），而手术出血量及住院时间分别为（30.36±11.58）ml、（4.16±0.66）d，与对照组的（45.26±20.49）ml、（7.16±1.82）d 比较差异有统计学意义（P ＜0.05）；观察组并发症总发生率及再出血率分别为5.88％、23.52％，与对照组的23.52％、35.29％比较差异有统计学意义（P ＜0.05）。结论鼻镜下电凝及手术治疗老年顽固性鼻出血疗效确切，安全性高，具有较高的临床应用价值。Objective To compare the clinical efficacy of different operation on elderly intracta-ble epistaxis. Methods Sixty-eight patients with elderly intractable epistaxis from July 2012 to November 2014 were randomly divided into observation group and control group with 34 cases in each group. The observation group were treated by electrocoagulation or operation under nasal endoscope while the control group received routine packing of anterior and post nasal. The clinical curative effect,adverse reactions and the rate of rebleeding of the two groups were compared. Results The total effective rate of the ob-servation group was 97. 06% ,which was significantly higher than that of the control group（67. 65% ,P〈 0. 05）. The operative time of the observation group was（19. 85 ± 3. 06）min,which was significantly longer than（10. 36 ± 2. 69）of the control group（P 〈 0. 05）,while the amount of intraoperative bleeding and hospitalization time respectively was（30. 36 ± 11. 58）ml and（4. 16 ± 0. 66）d,which were signifi-cantly lower than（45. 26 ±20. 49）ml and（7. 16 ± 1. 82）d of the control group（P 〈 0. 05）. The total oc-currence rates of complications and rebleeding in the observation group were 5. 88% and 23. 52%,which were significantly lower than 35. 29% and 23. 52% of the control group（P 〈 0. 05）. Conclusions The curative effect of nasal endoscopic electrocoagulation and operation on patients with elderly intractable epi-staxis is definite and with high safety and high value in clinical application.
