摘  要:[目的]观察低频电针(electracupuncture,EA)对2型糖尿病神经痛(diabetic neuropathic pain,DNP)大鼠背根神经节(dorsal root ganglion,DRG)降钙素基因相关肽(calcitonin gene-related peptide,CGRP)的干预作用,以探讨低频EA对2型DNP的镇痛机制。[方法]将50只SD大鼠用完全随机分组法分为正常对照组和高脂高糖组,分别用常规饲料和高脂高糖饲料喂养5周,监测2组大鼠体重(body weight,BW)、空腹血糖(fasting plasma glucose,FPG)、空腹胰岛素(fasting plasma insulin,FINS)。出现胰岛素抵抗(insulin resistance,IR)的大鼠继以小剂量链脲佐菌素(streptozotocin,STZ)腹腔注射1次,注射2周后分别测FPG、热痛阈(paw withdrawal latency,PWL)。将DNP模型制备成功的大鼠根据PWL采用随机区组分组法分为DNP组、2Hz EA组。EA干预于STZ注射后2周开始,每天1次,共7次,选取双侧"足三里"、"昆仑"穴。检测大鼠PWL,用免疫荧光法检测L6 DRGCGRP的表达情况。[结果]高脂高糖组大鼠于第5周BW、FINs增加(P<0.01,P<0.01),FPG未改变;第7周(即STZ注射2周后)FPG上升(P<0.01),PWL下降(P<0.01);DNP大鼠L6 DRG CGRP表达上调(P<0.01);2Hz EA上调DNP大鼠PWL(P<0.01),降低L6 DRG CGRP的阳性细胞表达水平(P<0.05)。[结论]低频EA对2型DNP模型大鼠有良好的镇痛作用,可能与其抑制DRG CGRP的表达有关。[Objective]To investigate the mechanism underlying the analgesic effect ofelectroacupuncture(EA) with low frequency on type Ⅱ diabetic neuropathic pain(DNP) by observing its effect on calcitonin gene-related peptide(CGRP) in dorsal root ganglion(DRG).[Methods] 50 SD rats were randomly divided into two groups: Normal group and Model group, fed with normal diet and high fat-glucose diet respectively. The changes of body weight(BW),fasting plasma glucose(FPG), fasting plasma insulin(FINs) and paw withdrawal latency(PWL) were determined.The rats in Model group were fed on high fat-glucose diet for 5 weeks to induce insulin resistance(IR). Rats with IR were further injected intraperitoneally with streptozotocin(STZ) for one time.FPG and PWL were determined 2 weeks after STZ injection. Rats with DNP were then divided into DNP group and 2Hz EA group. 2Hz EA was applied to bilateral ST36( "Zusanli") and BL60( "Kunlun") once daily for consecutive 7 days from 2 weeks after STZ injection. PWL was measured, and L6 DRG CGRP expression was determined by immunofluorescence method. [Results]After high fat-glucose diet for 5 weeks, rats' BW and FINs were significantly increased(P〈0.01,P〈0.01,respectively),and no significant change in FPG was found.2 weeks after STZ injection, FPG was significantly increased(P〈0.01),and PWL was significantly decreased(P〈 0.01).The expression of L6 DRG CGRP was upregulated in DNP rats(P〈 0.01). 2 Hz EA upregulated PWL and decreased CGRP expression in L6 DRG of DNP rats(P〈0.01, P〈0.05, respectively).[Conclusion]EA with low frequency has an analgesic effect on type ⅡDNP,which may be related to its inhibition on DRGCGRP expression.
