玉屏风颗粒联合盐酸氮卓斯汀喷鼻剂治疗过敏性鼻炎的临床研究

摘　　要：目的探讨玉屏风颗粒联合盐酸氮卓斯汀喷鼻剂治疗过敏性鼻炎的临床疗效。方法选取北京市顺义区医院2013年6月—2015年5月收治的过敏性鼻炎患者160例,随机分成对照组和治疗组,每组各80例。对照组患者给予盐酸氮卓斯汀鼻喷雾剂治疗,每侧鼻孔2喷/次,每日早晚各1次。治疗组在对照组基础上口服玉屏风颗粒,5 g/次,3次/d。两组患者均连续治疗4周。观察两组患者临床疗效,比较治疗前和治疗2、4周后两组的喷嚏、流涕、鼻痒、鼻堵评分、血清Ig E、Th1/Th2水平和炎性因子白介素-4(IL-4)、IL-6、IL-10水平的变化。结果治疗后,对照组和治疗组总有效率分别为78.75%、98.75%,两组比较差异有统计学意义(P<0.01)。治疗2、4周后,两组患者喷嚏、流涕、鼻痒、鼻堵评分均较治疗前显著降低,同组治疗前后差异有统计学意义(P<0.01);且治疗组的改善程度优于同期对照组,两组比较差异有统计学意义(P<0.01)。两组患者血清Ig E以及IL-4、IL-6、IL-10水平均较治疗前显著降低,Th1/Th2水平较治疗前显著升高,同组治疗前后差异有统计学意义(P<0.01);且治疗组这些观察指标的改善程度优于同期对照组,两组比较差异有统计学意义(P<0.01)。治疗组随访3、6个月的复发率分别为8.75%、16.25%,显著低于同期对照组,两组比较差异有统计学意义(P<0.01)。结论玉屏风颗粒联合盐酸氮卓斯汀喷鼻剂治疗过敏性鼻炎的临床疗效较好,可有效降低症状评分,改善相关因子水平,值得临床推广应用。Objective To investigate the clinical effect of Yupingfeng Granules combined with Azelastine Hydrochloride Nasal Spray in treatment of allergic rhinitis. Methods Patients（160 cases） with allergic rhinitis in Beijing Shunyi District Hospital from June 2013 to May 2015 were randomly divided into control and treatment groups, and each group had 80 cases. The patients in the control group were given Azelastine Hydrochloride Nasal Spray both nostril 2 spouts/time, morning and evening for 1 d. The patients in the treatment group were po administered with 5 g/time, three times daily. Both groups were treated for 4 weeks. After treatment, the efficacy was evaluated, changes of sneezing, runny nose, nasal itching, and nasal congestion scores, Ig E, Th1/Th2, inflammatory factor interleukin-4（IL-4）, IL-6, and IL-10 in two groups were compared. Results After treatment, the total effective rate of control and treatment groups were 78.75% and 98.75%, respectively, and there were differences between two groups（P〈0.01）. After treatment for 2 and 4weeks, sneezing, runny nose, nasal itching, and nasal congestion scores in two groups were significantly reduced, and the difference was statistically significant in the same group（P〈0.01）. And these indicators in treatment group improved better than those in the control group, with significant difference between two groups（P〈0.01）. Ig E, IL-4, IL-6 and IL-10 levels in two groups were significantly reduced, while Th1/Th2 was significantly increased, and the difference was statistically significant in the same group（P〈0.01）. The improvement of detection factor in the treatment group was better than those in the control group, and there were differences between two groups（P〈0.01）. The recurrence rate in treatment group after followed up for 3 and 6 months were 8.75% and 16.25%, respectively, and they were lower than those in control group at the same treatment time with significant difference（P〈0.01）.
