双切口双侧钢板治疗SchazkerⅤ,Ⅵ型骨折的疗效分析

摘　　要：目的:探讨双切口双侧钢板法与传统手术治疗Schazker Ⅴ、Ⅵ型骨折的临床疗效及安全性。方法:回顾性分析2007年6月-2012年6月在我院行手术治疗的50例Schazker Ⅴ、Ⅵ型骨折患者的病历资料,根据手术方法不同将患者分为治疗组及对照组。治疗组25例采用双切口双侧钢板治疗,对照组25例采用锁定钢板内固定治疗。观察并比较两组的临床疗效及膝关节功能评分变化。结果:治疗组住院时间、TPA、PSA、并发症发生率均低于对照组,差异有统计学意义(t=-2.889、-2.220、-2.377、4.348,P<0.05);治疗后两组Rasmussen分级优良率均高于治疗前(P<0.05),治疗组优良率高于对照组(P<0.05)。结论:采用双切口双钢板内固定术治疗Schatzker Ⅴ、Ⅵ型胫骨平台骨折患者的临床疗效明显优于传统手术方法,可以显著降低术后并发症的发生率及住院时间。Objective： To investigate the clinical effect and safety of double incision double plate internal fixation and traditional surgical method on schazker Ⅴ,Ⅵ fracture. Methods： A retrospective study was performed on 50 cases of patients treated in our hospital with tibal plateau fracture schazker Ⅴ,Ⅵ from June 2009 to June 2014 and divided into the double incision double plate internal fixation group（treatment group） and the traditional surgical method group, 25 cases in each group. The clinical efficacy and change of knee function score were compared between two groups. Results： Compared with the control group, the hospitalization time, TPA, PSA,incidence of complications weresignificantly lower in the treatment group（t=-2.889,-2.220,-2.377, 4.348, P〈0.05）. Compared with before treatment, the excellent rate of Rasmussen grading of both groups were higher（P〈0.05）. Compared with the control group, the excellent rate of treatment group after treatment was higher（P〈0.05）. Conclusion： For tibal plateau fracture schazker Ⅴ,Ⅵ patients, the clinical efficacy of double incision double plate internal fixation was better than traditional surgical method, which could decrease the incidence of complications and hospital stay.
