摘 要:目的:探究原发性高血压患者血尿酸水平与左心室肥厚的相关性。方法:收集2012年6月至2013年12月,在安贞医院高血压科诊治的原发性高血压患者581例为研究对象,并行超声心动图测定心脏各腔室内径,计算左心室质量指数(LVMI),根据左心室质量指数(LVMI)≥125g/m^2(男),LVMI≥110g/m^2(女)分为高血压左心室肥厚组和非肥厚组(对照组),分析两组患者基本临床资料,再采用多因素Logistics回归分析的方法,进行UA与LVMI的相关性分析。结果:1.肥厚组与对照组比较,肥厚组尿酸[(453.72±120.63)vs.(349.23±87.39)μmol/L,P=0.00],收缩压[(156.02±22.34)vs.(148.19±22.06)mm Hg,P=0.00)]、(1mm Hg=0.133k Pa)脉压[(52.86±14.08)vs.(47.20±15.52)mm Hg,P=0.00]、LVMI[133.51(124.79,146.26),P=0.00]均高于对照组,差异有统计学意义(P<0.05);2两组不同性别的高血压患者的血尿酸水平比较:在肥厚组中,男性血尿酸水平高于女性(t=2.73,P=0.01),在对照组中,男性血尿酸水平也高于女性(t=4.57,P=0.00),差异均有统计学意义(P<0.05)。3影响左心室肥厚的多因素Logistic回归分析显示,进一步校正了年龄、性别、高血压病程等心血管危险因素,结果显示血尿酸仍是高血压患者左心室肥厚的独立危险因素(OR=1.03,P=0.00),性别(OR=2.51,P=0.00)和脉压(OR=1.02,P=0.03)也与左心室肥厚密切相关(P<0.05)。结论:尿酸与高血压左心室肥厚存在相关性,随着高血压患者的尿酸水平增加,其左心室肥厚显著增加。Objective: This article aim to explore the relation between blood uric acid level and left ventricular hypertrophy in EH patients. Methods :Intact data of 581 cases of patients with essential hyperten- sion (EH) were collected from June 2012 to December 2013 at Anzhen hospital hypertension including blood pressure, height weight, blood uric acid and other biochemical indicators, it worth to be mentioned, all bio- chemical index were measured after fasting 12 hours. According to the level of left ventricular mass index ( LV- MI) which was calculated via echo-cardiogram, all cases were divided into hypertensive left ventricular hypertrophy group (LVMI 〉t 125g/m^2 , male) which also is the control group, and hypertensive left ventricular hypertrophy group (LVMI ≥ 110g/m^2, female). Analysis of basic clinical data of two groups of patients, and then adopt the method of multi-factor logistics regression analysis, to analyze the correlation between UA and LVMI. Results: 1. Hypertrophy group compared with control group, uric acid [ (453.72 ± 120. 63 )vs. (349. 23± 87.39) μmol/L,P = 0. 00], systolic blood pressure [ ( 156. 02 ± 22. 34) vs. ( 148. 19 ± 22. 06) mmHg, P = 0. 00) ], pulse pressure[ (52. 86 ±14. 08 ) vs. (47. 20 ± 15.52 ) mmHg, P = 0. 00 difference was statistically significant. 2. Male blood uric acid level of hypertrophy group was obviously higher than hypertrophy group of women, with noticeable statistical significance ( P = 0. 03 ). 3. Influence of left ventricular hypertrophy multivariable logistic regression analysis showed that, with a further correction of age, sex, duration of hypertension and other cardiovascular risk factors, the results show that blood uric acid is still independent risk factors for hypertension patients with left ventricular hypertrophy ( OR = 1. 029, P = 0. 00, gender ( OR = 2. 51, P = 0. 00) and pulse pressure ( OR = 1.02, P = 0. 03 ) are closely related with left ventricular hypertrophy ( P 〈 0. 05 �
