金天格胶囊联合西药治疗膝骨关节炎的疗效观察及其对IL-1β、MMP-1表达的影响

摘　　要：目的:观察中药金天格胶囊联合依托考昔治疗膝骨关节炎患者的疗效及其对白细胞介素1β(IL-1β)、基质金属蛋白酶1(MMP-1)表达的影响。方法:124例膝骨关节炎患者,随机均分为观察组和对照组,观察组应用金天格胶囊联合依托考昔进行联合治疗,对照组单独使用依托考昔治疗;治疗12 W后评价临床疗效,并观察治疗前后骨关节炎指数(WOMAC)评分变化,应用ELISA法检测血清IL-1β、MMP-1含量的变化。结果:观察组临床总有效率为90.3%,显著优于对照组的77.4%(P<0.05);与治疗前比较,2组患者WOMAC(疼痛、关节僵硬、日常活动、综合)评分显著减少,统计学处理具有显著意义(P<0.05);与对照组比较,观察组患者WOMAC评分显著减少,差异具有统计学意义(P<0.05)。2组患者血清IL-1β、MMP-1表达水平显著下调,与治疗前比较差异均具有统计学意义(P<0.05);观察组效果较对照组显著,具有统计学意义(P<0.05)。不良事件发生率组间比较差异不具有统计学意义(P>0.05)。结论:中药金天格胶囊联合依托考昔治疗膝骨关节炎效果显著,下调外周血清IL-1β、MMP-1的表达是其作用机制之一。Objective： To investigate the effects of traditional Chinese medicine Jintiange Capsule combined with etoricoxib on patients with knee osteoarthritis,and influences on expressions of serum IL- 1β,MMP- 1.Methods： 124 cases of knee osteoarthritis were randomly divided into control group（ etoricoxib treatment） and observation group（ etoricoxib ＋ Jintiange capsule）,each group was given the corresponding medicine for 12 weeks.. The curative effect was evaluated,WOMAC score of each group patients was recorded before and after treatment,ELISA method was applied to determine serum IL- 1β and MMP- 1 changes. Results： The efficiency rate of observation group（ 90. 3%） was significantly higher than that of control group（ 77. 4%）（ P〈0. 05）.The pain score,anchylosis score,daily activities score and colligation score were reduced in the two groups,those were smaller in obseration group compared with those in the control group（ all P〈0. 05）. The serum IL-1β and MMP- 1 levels were significantly decreased in the two groups after treatment,the levels in observation group were lower than those of the control group（ P〈0. 05）. There was no statistical difference on adverse events in the two groups（ P〈0. 05）. Conclusion： Jintiange Capsule combined with etoricoxib can effectively treat knee osteoarthritis,reducing the levels of IL- 1β and MMP- 1 maybe one of the mechanisms.
