中药竹罐泻血疗法联合中药内服治疗膝骨关节炎的临床研究

摘　　要：目的:观察中药竹罐泻血疗法联合中药内服治疗膝骨关节炎的临床疗效并探讨其可能的作用机制。方法:符合入组标准的65例患者随机分为2组,观察组采用中药组方煎汤煮罐,取穴犊鼻、内膝眼、梁丘及痛点刺络拔罐泻血,每W1次,同时依据辨证给予中药口服;对照组仅给予辨证中药口服,疗程均为3个月。统计疗效,并对2组患者进行数字疼痛量表(NRS)和描述疼痛量表(VRS)评分,观察治疗前后的变化;采用ELISA法检测2组患者治疗前后血清白细胞介素-1(IL-1)含量的变化。结果:观察组临床总有效率为93.9%,明显优于对照组之71.9%,差异具统计学意义(P<0.05);治疗后2组患者NRS、VRS评分均较治疗前明显降低,观察组效果明显优于对照组,差异均具有统计学意义(P<0.05);2组治疗后血清IL-1水平均显著降低,观察组较对照组明显。结论:中药竹罐泻血疗法联合中药内服治疗能够有效改善膝骨关节炎患者的临床症状,下调血清IL-1水平是其作用机制之一。Objective： To observe the curative effect of knee osteoarthritis treated with shallow puncture with Chinese herbs- bamboo for cupping combining with oral Chinese medicine decoction,and explore the mechanism. Methods： 65 Cases with knee osteoarthritis were divided into observation and control groups,observation group was given shallow puncture on ＂Tubi（ St35） ＂＂Hsiyen（ Medial,Extra） ＂＂Liangchiu（ St34） ＂points and pain spots with Chinese herbs decoction boiling bamboo jar for cupping,once a week,meanwhile oral Chinese medicine decoction according to differentiation of syndromes every day,the control group was only given Chinese medicine decoction orally,treatment course was three months. After treatment,the curative effect was taken statistical evaluate,the scores of NRS and VRS scale were recorded before and after treatment in the two groups,serum IL- 1 content was determined by ELISA. Results： The total effective rate of observation group was 93. 9%,that was much better than that of control which was 71. 9%,the difference of the two groups was significant（ P〈0. 05）; the scores of NRS and VRS scale in the two groups were obviously decreased to compare with pro- treatment,the effects of observation group were better than those of control group,the differences were significant（ P〈0. 05）; the serum IL- 1 level was reduced in the two group after treatment,the effect of observation group was more obvious than that of control. Conclusion： Shallow puncture with Chinese herbs- bamboo for cupping combining with oral Chinese medicine decoction can improve clinical symptoms significantly,down- regulating serum IL- 1 level is one of the mechanism.
