摘  要:目的:建立一个以护理工作量为科学依据的动态急诊护理人力配置模型。方法:采取观察法一对一跟班记录护理工时,同时评估计算国家早期预警评分系统(NEWS)评分和改良NEWS评分分值,将患者直接护理工时分别与NEWS评分和改良NEWS评分进行相关分析,建立一个护理工时与改良NEWS评分的回归方程,从而建立一个科学、动态的急诊护理人力配置模型。结果:患者直接护理工时与NEWS评分的相关系数为0.659(P<0.01),与改良NEWS评分的相关系数为0.701(P<0.01);患者直接护理工时=91.765+4.856×改良NEWS评分;病区配置护士人数=∑∑ (91.765+4.856×改良NEWS评分)×0.005 22;每班配置护士人数=∑ (91.765+4.856×改良NEWS评分)×0.003 48。结论:基于改良NEWS评分建立的急诊护理人力配置模型经统计学证明按照预测值进行护理人力配置与临床实际需求相符。Objective:To establish a dynamic emergency nursing manpower allocation model based on nursing workload.Method:The nursing time was recorded by one-on-one observation,while NEWS score and modified NEWS score were evaluated.The correlation between the direct nursing time and the NEWS score as well as modified NEWS score was analyzed.A regression equation about nursing time and modified NEWS score was established,so as to develop a emergency nursing manpower allocation model that was scientific and dynamic.Result:The correlation coefficient between the direct nursing time and the NEWS score was 0.659(P〈0.01),and the correlation coefficient between the direct nursing time and the modified NEWS score was 0.701(P〈0.01).Patient direct nursing hours=91.765+4.856×improved NEWS score.The number of nurses in the configuration=∑ ∑ (91.765+4.856×improved NEWS score)×0.005 22.The number of nurses in each scheduling =∑ (91.765+4.856×improved NEWS score)×0.003 48.Conclusion:The emergency nursing manpower allocation model based on the modified NEWS score is proved to be in compliance with clinical practice by statistical analysis.
