摘 要:目的研究丝裂原活化蛋白激酶p38(p38 MAPK)在慢性阻塞性肺疾病(COPD)患者外周血单个核细胞中的表达情况及其与气流受限的相关性。方法选择确诊的急性加重期COPD(AECOPD)患者50例作为实验组,根据肺功能检测,分为重-极重度组(A组)26例和轻-中度组(B组)24例。选择同期门诊健康受试者25例为对照组,采集各组受试者外周血,运用PCR荧光检测法测定P38 MAPK mRNA的表达,酶联免疫吸附法(ELISA)检测外周血白细胞介素(IL)-6和IL-8含量。结果实验组患者外周血单个核细胞核内p38 MAPK mRNA表达和血清IL-6、IL-8含量均高于对照组(P<0.05);A组患者外周血单个核细胞核内p38 MAPK mRNA的表达和血清IL-6、IL-8含量显著高于B组(P<0.05);AECOPD患者外周血单个核细胞核内p38 MAPK mRNA表达随1 s用力呼气容积(FEV1)占预计值升高而下降(r=-0.91,P<0.05)。结论 p38 MAPK、IL-6、IL-8参与了COPD的炎症反应,且同COPD患者的气流受限程度呈正相关。Objective To investigate the correlation between expression of p38 MAPK in peripheral blood mononuclear cells of pa= tients with chronic obstructive pulmonary diseases(COPD)and airflow obstruction. Methods 50 patients diagnosed as acute exacerbation of COPD (AECOPD)were involved as experimental group. Based on pulmonary function tests, they were divided into Group A( severe to extreme- ly severe)and Group B( mild to moderate). 26 and 24 patients were assigned to Group A and Group B respectively. 25 healthy subjects were selected from the outpatient service. Peripheral blood from subjects in each group was collected. The expression of mRNA of p38 MAPK was determined by PCR fluorescence detection while the concentrations of IL-6 and IL-8 in peripheral blood were measured via ELISA. Results The expression of p38 MAPK mRNA in nuclei of peripheral blood mononuclear cells and the concentrations of IL-6 and IL-8 in serum were higher in patients from the experimental group than the control group(P〈O. 05). The expression of p38 MAPK mRNA in nuclei of peripheral blood mononuclear ceils and the concentrations of IL-6 and IL-8 in serum of Group A Were significantly higher than those of Group B (P〈 0. 05). The expression of nuclear mRNA P38 MAPK in peripheral blood mononuclear cells in patients with AECOPD decreased as FEV1% pred increased and the correlation coefficient( r = -0.91, P〈O. 05 ). Conclusions p38 MAPK, IL-6 and IL-8 are involved in the inflammato- ry response of COPD and are positively correlated with airflow obstruction in COPD patients.
