成都市某医院乳腺癌患者住院费用影响因素分析

摘　　要：目的:分析乳腺癌患者住院费用的主要影响因素,为降低住院费用提供依据。方法:选取成都市某医院2011年1月-2013年12月乳腺癌住院患者的住院情况和费用信息,采用单因素方差分析和多重逐步回归分析对住院费用的影响因素进行研究。结果:患者平均住院费用为32803.37元,其中,药费为28711.61元,占平均总费用的87.53%,是乳腺癌患者住院治疗的最大费用构成;住院次数和天数对住院费用的影响具有统计学意义(均P<0.05);在不同支付方式中,省医保患者住院费用最高,城乡居民保险患者住院费用最低;不同临床分期患者的住院费用也存在较大差异,Ⅳ期患者费用为0-Ⅰ期患者费用的2.48倍。结论:有效降低乳腺癌患者的药费是控制日益增长的乳腺癌住院费用的关键。通过加强医院管理,从降低住院天数、规范合理用药和改变支付方式等方面入手,可最终降低乳腺癌住院费用,减轻患者和社会的经济负担。Objective： To analyze the hospitalization expense of patients with breast cancer and to find out the main influencing factors of it,to provide reference for reducing the hospitalization expencse of patients with breast cancer. Methods： The information about hospitalization and the expense of inpatients with breast cancer was selected in a hospital in Chengdu from January 2011 to December 2013. ANOVA and multiple regression analysis were used to analyze the influencing factors of hospitalization expense. Results： Totally 2729 valid cases were collected,and the mean expense of hospitalization was 32 803. 37 RMB,of which drug expense which was the largest cost component was 28 711. 61 RMB,accounting for 87. 53%. Number of hospitalization and days of hospitalization were important factors affecting the hospitalization expense of patients with breast cancer in this hospital. The hospitalization of patients hospitalizing more than 10 times was 12. 81 times higher than that of patients hospitalizing 1 time. In different methods of payment,the expense of patients with provincial health insurance was the highest,and that of patients with resident insurance was the lowest. The expense of patients with different clinical stages were quite different,and the Ⅳ patients cost 2. 62 times than that of 0- I patients. Conclusion： To control the increasing expense of patients with breast cancer,effectively reducing drug expenses of patients could be a critical path. We could ultimately reduce the hospitalization expense of patients with breast cancer as well as the economic burden of patients and society by shortening hospital stay,rationally regulating drug use,and changing the mode of payment.
