摘 要:目的观察通腑益气汤对脓毒症模型大鼠血清TNF-α、IL-lβ及ICAM-1水平影响和肠组织的病理改变。方法将雄性SD大鼠按随机数字表法分为模型组、氧氟沙星组、通腑益气汤组,每组12只。采用经典盲肠结扎穿孔法(CLP)制备大鼠脓毒症模型,氧氟沙星组采用灌胃氧氟沙星16mg/kg,通腑益气汤组在此基础上给予通腑益气汤水煎剂(含生药1g/m L)灌胃,每天1次,连续3天,模型组用等量生理盐水灌胃。检测各组大鼠血清TNF-α、IL-lβ及ICAM-1水平和光学显微镜下观察肠组织病理改变并进行评分。结果模型组大鼠血清TNF-α、IL-lβ及ICAM-1分别为(95.26±34.73)ng/m L、(30.83±12.32)pg/m L、(914.72±82.13)pg/m L,氧氟沙星组分别为(78.46±30.24)ng/m L、(24.72±10.53)pg/m L、(790.69±60.24)pg/m L,通腑益气汤组分别为(60.38±21.81)ng/m L、(19.31±8.62)pg/m L、(681.69±52.62)pg/m L,与模型组和氧氟沙星组比较,通腑益气汤组大鼠血清TNF-α、IL-lβ、ICAM-1水平明显降低(P<0.05);模型组、氧氟沙星组、通腑益气汤组肠组织损伤评分分别为(4.21±0.61)分、(3.56±0.32)分、(2.84±0.27)分,与模型组和氧氟沙星组比较,通腑益气汤组肠组织损伤病理学评分最低(P<0.05)。结论通腑益气汤对脓毒症模型大鼠肠黏膜的保护作用可能与其抑制TNF-α、IL-1β过度表达,进而下调ICAM-1水平有关。Objective To investigate the effects of Tongfu Yiqi decoction on serum levels of TNF-α, IL-lβ, and ICAM-1 and intestinal injury in rats with sepsis. Methods Thirty-six male SD rats were randomly divided into 3 groups: model group, ofloxacin group and Tongfu Yiqi decoction group (Chinese medicine group), with 12 rats in each group. Sepsis model was reproduced by cecal ligation and puncture (CLP). Ofloxacin group received ofloxacin (16mg/kg) via gastric infusion, Chinese medicine group received Tongfu Yiqi decoction (crude drug 1g/mL) with gastric infusion, once daily for 3 d, model group received an equal dose of normal saline in the same way. The serum concentrations of TNF-α, IL-lβ and ICAM-1 in each group were determined, the intestinal histopathological changes were evaluated and scored by light microscope after treatment. Results The serum concentrations of TNF-α, IL-lβ and ICAM-1 in model group were 95.26±34.73ng/mL, 30.83±12.32pg/mL, 914.72±82.13pg/mL; those in ofloxacin group were 78.46 ±30.24ng/mL, 24.72 ±10.53pg/mL, 790.69 ±60.24pg/mL; those in Chinese medicine group were 60.38±21.81ng/mL, 19.31±8.62pg/mL, 681.69±52.62pg/mL; compared to other two groups, a significant decrease in those index in Chinese medicine group was noted (P〈0.05). The scores of intestinal injury in model group, ofloxacin group, and Chinese drugs group were 4.21±0.61, 3.56±0.32, 2.84±0.27, respectively, with a significant dif-ference among them(P〈0.05). Conclusion Tongfu Qiyi decoction might suppress the release of inflammatory medi-ators(TNF-α, IL-lβ) and down-regulate ICAM-1 response to ameliorate the intestinal injury.
