161例冠状动脉CT血管造影正常者的左心室心肌强化程度分析

摘　　要：目的分析64排螺旋CT血管造影(CTA)冠状动脉(简称冠脉)正常者的左心室心肌强化程度,为应用CTA判断冠脉狭窄患者左心室心肌缺血(心肌强化程度减低)提供参考。方法选取我院近2年行64排螺旋CT心脏冠脉成像中冠脉正常者161例为观察对象,观察其室间隔中部、左心室前壁中部、底壁中部、心尖部4个感兴趣区(ROI)测量其平扫及增强时的心肌密度(CT值),分析其左心室心肌强化情况。结果 161例观察对象的各ROI强化值有一定差别,由高到底依次为左心室底壁、室间隔中部、左心室前壁、左心室尖部;各ROI左心室心肌强化值女性高于男性(均P<0.01)。结论64排螺旋CT心脏冠脉成像中冠脉正常者的左心室心肌强化程度的初步参考范围为(55.15±9.65)^(67.28±8.36)Hu,可为进一步应用CTA判断冠心病患者左心室心肌缺血情况提供参考。Objective To analyze left ventricular myocardium enhancement of the cases with normal coronary artery（CA） in 64-slice CT angiography（CTA）, providing reference for using CTA to judge left ventricular myocardial ischemia（reduced ventricular myocardium enhancement） in the patients with coronary artery stenosis. Methods 161 cases（male=93, female=68） with normal coronary artery in 64-slice CT angiography in the past two years were enrolled as observation object. Four regions of interest（ROI） including the middle part of interventricular septum, and the anterior wall, basal wall and apex of left ventricle were observed to measure myocardium density（CT value） in plain and enhanced CT and analyze left ventricular myocardium enhancement. Results There was difference in each ROI enhancement of 161 cases of observation subjects. ROI enhancement in the basal wall of left ventricle, the middle part of interventricular septum, the anterior wall and apex of left ventricle ranked from high to low. The range of left ventricular myocardium enhancement of RIO was from 55.15±9.65 Hu to 67.28±8.36 Hu. There was significant difference between male and female group in terms of four ROIs（P〈0.05）. The enhancement value of female in every ROI was significantly higher than that of male（P〈0.05）. Conclusion The possible normal reference range of left ventricular myocardium enhancement value in the cases with normal coronary artery in 64-slice CT angiography was（55.15±9.65）-（67.28±8.36） Hu, providing reference for using CTA to judge left ventricular myocardial ischemia in the patients with coronary heart disease.
