关节镜术联合中药外洗治疗创伤踝关节炎探讨

摘　　要：目的探讨关节镜清理术联合中药外洗治疗创伤性踝关节炎的疗效。方法收集2010年6月—2014年8月在我院骨科住院治疗的踝关节骨折术后TAA患者60例。按抽签法将患者分为联合组和单纯关节镜组,每组30例。单纯关节镜组仅用关节镜清理术治疗,联合组在使用关节镜清理术后用中药外洗,观察两组治疗前后美国足踝外科(AOFAS)踝-后足评分及临床疗效情况。结果治疗后联合组AOFAS评分明显高于单纯关节镜组(P<0.05)。联合组优良率明显高于对照组,差异有统计学意义(P<0.05)。结论关节镜清理术联合中药外洗可有效提高TAA患者的AOFAS评分,促进其踝关节功能改善,提高总体临床效果。Objective To investigate the clinical effect of arthroscopy combined with external washing of Chinese medicine in treating traumatic ankle arthritis（ TAA）. Methods 60 patients with TAA were collected in orthopedics department of our hospital from June 2010 to August 2014. They were divided into simple arthroscopic group and combination group by drawing lots,and there were 30 patients in each group. The simple arthroscopic group was only received arthroscopic debridement. The combination group was treated with external washing of Chinese medicine after arthroscopic debridement. The American Orthopaedic Foot Ankle Society（ AOFAS） ankle-hindfoot score and clinical curative effect of the two groups were observed. Results After treatment,the AOFAS score of the combination group was significantly higher than that of the simple arthroscopic group（ P〈0. 05）. The excellent and good rate of the combination group was significantly higher than those of the control group,and the difference was statistically significant（ P〈0. 05）. Conclusion Arthroscopic debridement combined with external washing of Chinese medicine can effectively improve the AOFAS score in patients with TAA and the function of ankle joint function,and raise the overall clinical effect.
