光动力疗法联合重组人干扰素α-2b治疗男性尿道口尖锐湿疣的临床研究

摘　　要：目的应用5-氨基酮戊酸光动力疗法(5-Aminolevulinic acid Photodynamic therapy ALA-PDT)与重组人干扰素α2b联合治疗男性尿道口尖锐湿疣(Condyloma acuminatum CA),观察局部病毒的清除情况及对复发的作用,探索治疗男性尿道口尖锐湿疣潜伏感染的新方法。方法男性尿道口CA患者90例,随机分为治疗组和对照组两组,治疗组患者45例,采用ALA-PDT与干扰素联合治疗;对照组患者45例,采用单纯ALA-PDT治疗。两组患者术前和术后7 d应用PCR法检测尿道口皮损区人乳头瘤病毒DNA(Human papilloma virus DNA,HPV-DNA),术后15 d,1、3和6个月进行门诊或电话随访,观察临床疗效。结果治疗组病毒清除率85.06%,对照组病毒清除率62.41%,治疗组和对照组清除率比较,差异有统计学意义(P<0.05);治疗组复发率6.67%,对照组复发率24.4%,两组复发率比较差异有显著意义(P<0.05)。结论 ALA-PDT与干扰素联合治疗可降低尖锐湿疣的复发率,对控制HPV的潜伏感染具有重要作用。Objective To treat male urethral mouth condyloma acuminatum （CA） jointly with photodynamic therapy （ ALA - PDT） and recombinant human interferon a-2b, observe the clearance of virus and effect on local recurrence, and explore new treating methods for latent infection of male genital warts. Methods Totally 90 male patients of urethra CA were randomly divided into two groups ： the treatment group （ consisting of 45 patients given ALA-PDT and interferon jointly） and control group （consisting of 45 patients given ALA-PDT solely）. Detection of human papillomavirus DNA （HPV-DNA） was performed on the urethra lesion areas with PCR before and after the treatment.Results The virus removal rate was 85.1% for the treatment group and 62.4% for the control group, indicating a difference that was statistically significant （P〈0.05）. The relapse rate was 6.67% for the treatment group and 24. 4% for the control group, indicating a difference that was statistically significant （P〈0.05）. Conclusions ALA-PDT combined with interferon treatment of genital warts and recurrent HPV latent infection control have an important role.
