摘  要:目的分析人类正常肾组织和蛋白尿患者的临床表现和肾脏病理特征,以探讨蛋白尿的临床意义。方法对10名正常患者及83例蛋白尿患者进行回顾性收集、分析其临床、病理资料,进一步分析蛋白尿患者临床表现与病理分级分布情况。结果 (1)临床表现:蛋白尿患者分级各组血肌酐、CRR比较有统计学意义(P<0.05);而IgA、C3比较无统计学意义(P>0.05)。(2)病理分析:病理分类中Ⅲ类与其他病理类型比较血肌酐、C反应蛋白有统计学意义(P<0.05);而各类病理类型之间比较IgA、补体C3无统计学意义(P>0.05)。结论 (1)蛋白尿可加速或导致肾小球疾病的进展。(2)不同病理类型肾小球疾病尿蛋白排出量与肾病进展的关系,尿蛋白排出越多肾病进展越快。(3)蛋白尿促进肾小球疾病的进展与肾小球基础病理类型有关。Objective Analysis the Clinical manifestations and the Pathological features of human normal nephridial tissue and in patients' nephridial tissue with proteinuria to discuss the the clinical significance of proteinuria.Method To analyze restrospectively of 10 normal patients and 83 patients with proteinuria clinical and pathological data,then analyze the clinical manifestation,the grade and the distributions of renal pathology.Results(1)Clinical manifestations:In patients with proteinuria group,compared with the serum creatinine,CRP clinical data,difference was statistically significant(P〈0.05);the complement C3 and the IgA have no significant difference(P〉0.05).(2)Pathological analysis:In the Pathological Classification,ClassⅢtype compared with other pathological,serum creatinine,CRP was statistically significant differently(P〈0.05);the complement C3 and the IgA have no significant difference(P〉0.05).Conclusions(1)Proteinuria can accelerate or cause glomerular disease progression.(2)In the different pathological types of glomerular diseases,relationship of urinary protein excretion and renal disease progression:urinary protein excretion was more faster progression of renal disease.(3)Proteinuria promote glomerular disease progression based on pathological type.
