西医综合疗法联合保肾消浊汤治疗老年难治性肾病综合征的临床观察及安全性评价

摘　　要：目的观察西医综合疗法联合保肾消浊汤治疗老年难治性肾病综合征的临床疗效及安全性。方法将84例老年难治性肾病综合症患者随机分为2组,对照组42例予西医综合疗法治疗,治疗组42例在对照组治疗基础上予保肾消浊汤治疗。2组均4周为1个疗程,连续治疗6个疗程。观察2组治疗前后实验室指标[24 h尿蛋白定量、血浆白蛋白(Alb)、尿素氮(BUN)及血肌酐(Cr)]变化,治疗后对2组进行疗效评定,记录2组治疗期间不良反应发生率。结果 2组治疗后24 h尿蛋白定量、BUN及Cr均低于本组治疗前(P<0.05),Alb高于本组治疗前(P<0.05)。2组治疗后以上实验室指标比较差异均有统计学意义,治疗组24 h尿蛋白定量、BUN及Cr均低于对照组(P<0.05),Alb高于对照组(P<0.05)。治疗组总有效率90.48%,对照组69.05%,2组总有效率比较差异有统计学意义(P<0.05),治疗组疗效优于对照组。治疗组不良反应发生率7.14%,对照组23.81%,治疗组不良反应发生率低于对照组(P<0.05)。结论西医综合疗法联合保肾消浊汤治疗老年难治性肾病综合征疗效确切,安全可靠,可有效降低激素抵抗及依赖,提高免疫抑制剂功效,降低不良反应。Objective To observe the clinical efficacy and safety evaluation of western combining therapy combined with Baoshen- xiaozhuo decoction on elderly refractory nephropathy syndrome. Methods 84 subjects with elderly refractory nephropathy syndrome were randomly divided into two groups,42 cases in each group. The control group received western combining therapy,and the treatment group received Baoshen- xiaozhuo decoction on the basis of control group treatment. The laboratory indexes,including 24 urine protein quantitation,plasma albumin（ Alb）,blood urea nitrogen and serum creatinine before and after treatment were observed in two groups. The curative effect was evaluated after treatment. The clinical symptom and adverse reaction during treatment were recorded in two groups. Results The 24 urine protein quantitation,blood urea nitrogen and serum creatinine were decreased in two groups（ P〈0. 05）,and the Alb was increased（ P〈0. 05）,with statistical differences. The 24 urine protein quantitation,blood urea nitrogen and serum creatinine after treatment in treatment group were lower than those in control group（ P〈0. 05）,and the Alb was higher（ P〈0. 05）. The total effective rate in treatment group（ 90. 48%） was superior to that in control group（ 69. 05%,P〈0. 05）. The occurrence rate of adverse reaction in treatment group（ 7. 14%） was lower than that in control group（ 23. 81%,P〈0. 05）. Conclusion Western combining therapy combined with Baoshen- xiaozhuo decoction has exact effects on the treatment of elderly refractory nephropathy syndrome,safe and reliable,can effectively reduce hormonal resistance and dependent,and enhance the efficiency of immunosuppressor,decrease adverse reaction.
