摘  要:目的观察腹舒胶囊对慢传输型便秘大鼠模型肠道传输功能、结肠壁病理结构及结肠肌间神经丛生长抑素(SOM)的影响,探讨腹舒胶囊治疗慢传输型便秘的作用机制。方法取72只SD大鼠,随机选取其中18只作为正常组,给予普通饲料喂养;其余大鼠给予添加大黄的饲料造模。将造模成功后大鼠随机分为模型组、腹舒胶囊组和自然恢复组各18只。腹舒胶囊组和自然恢复组分别给予腹舒胶囊和生理盐水灌胃2周。采用活性炭悬液推进法测定各组大鼠炭末推进长度,光镜及电镜观察各组动物结肠壁的病理及超微结构变化,进行结肠肌间神经丛SOM阳性神经免疫组织化学染色半定量分析。结果模型组炭末推进长度明显短于正常组(P<0.05);经光镜及电镜观察,模型组可见黏膜慢性炎症改变,神经元及平滑肌细胞呈明显退行性变化,肠壁大量嗜酸性粒细胞浸润;免疫组化染色显示模型组SOM免疫反应性明显低于正常组(P<0.05)。而腹舒胶囊组灌胃2周后炭末推进长度明显长于模型组和自然恢复组(P均<0.05),肠壁神经病变明显减轻,免疫组化染色显示SOM免疫反应性明显高于模型组和自然恢复组(P均<0.05)。结论长期应用蒽醌类泻剂对肠神经系统有明显损害作用,并可引起结肠肌间神经丛神经递质的变化。腹舒胶囊可促进结肠肌间神经丛神经递质数量及功能恢复,对于治疗慢传输型便秘有较好的疗效。Objective It is to observe the influence of Fushu capsule on intestinal transmission function,pathological structure of colon wall and the somatostatin( SOM) in the colonic muscular plexus of the rat model of slow transit constipation( STC),and explore the mechanism of the treatment of slow transit constipation. Methods 72 healthy adult SD rats were selected. 18 rats were randomly selected as the normal group,which were fed with normal diet. The rest of the rats were given the feed of rhubarb to produce STC model,and then randomly divided into the model group,the Fushu capsule group and the natural recovery group with 18 cases in each groups. The animals in Fushu capsule group and natural recovery were given Fushu capsule and physiological saline to fill the stomach respectively for 2 weeks. The length of carbon propulsion were measured by activated carbon suspension propulsion method,the pathological and ultrastructural changes of rat colonic wall were observed by light microscope and electron microscope,and the semi quantitative analysis of immunohistochemical staining of SOM positive nerve in the muscular plexus of colon was performed. Results The length of carbon propulsion in model group was significantly shorter than that in normal group( P 0. 05). The chronic inflammatory changes were discovered in the mucosa of model group,and the neurons and smooth muscle cells were obviously degenerative,a large number of Eosinophil infiltration in the intestinal wall; Immunohistochemical staining showed that the SOM of model group was significantly lower than that of normal group( P 0. 05). 2 weeks after filling the stomach,the carbon propulsion length in Fushu caspsule group was significantly longer than that in the model group and natural recovery group( all P 0. 05); the intestinal wall neuropathy was significantly reduced; Immunohistochemical staining showed that SOM immunoreactivity was significantly higher than that of model group and natural recovery group( all P 0. 05).
