声触诊组织量化成像技术对颈部转移性与非转移性淋巴结的鉴别诊断价值

摘　　要：目的探讨声触诊组织量化成像(VTIQ)技术对颈部转移性与非转移性淋巴结鉴别诊断价值。方法回顾性分析经病理证实的85例患者85个颈部肿大淋巴结的VTIQ图像资料。测量淋巴结的VTIQ剪切波速度(SWV),绘制受试者操作特征曲线(ROC),评估VTIQ对颈部转移性与非转移性淋巴结的鉴别诊断价值。结果 85个颈部肿大淋巴结中,转移性淋巴结44个,非转移性淋巴结41个(其中血液及淋巴系统疾病淋巴结24个,反应性增生淋巴结17个)。转移性淋巴结SWV值明显高于非转移性淋巴结SWV值(P<0.001)。转移性淋巴结与血液及淋巴系统疾病淋巴结比较,ROC曲线下面积(AUC)为0.943,SWV截断值为3.23m/s时,准确性88.24%、灵敏度88.64%、特异度87.50%;转移性淋巴结与反应性增生淋巴结比较,AUC为0.968,SWV截断值为3.27 m/s时,准确性90.16%、灵敏度88.64%、特异度94.12%。结论 VTIQ对鉴别颈部转移性与非转移性淋巴结有一定诊断价值。Objective To evaluate virtual touch tissue imaging quantification（VTIQ）for differentiating metastatic from non-metastatic cervical lymph nodes.Methods 85 patients with 85 pathologically proven metastatic（44）and nonmetastatic（41）cervical lymph nodes underwent VTIQ examinations.Shear wave velocities（SWVs）of metastatic and non-metastatic lymph nodes were measured.The receiver operating characteristic curve（ROC）was used to assess the value of VTIQ in differentiating metastatic and non-metastatic cervical lymph nodes.Results The SWVs between metastatic and non-metastatic cervical lymph nodes were significantly different（P0.001）.Comparison between the metastatic lymph nodes and the lymphatic system diseases,the AUC was 0.943 and the SWV cut-off value was 3.23m/s,the accuracy was 88.24%,sensitivity was 88.64%,specificity was 87.50%,respectively.Comparison between the metastatic lymph nodes and reactive lymphoid hyperplasia,the AUC was 0.968 and the SWV cut-off value was 3.27m/s,the accuracy was 90.16%,sensitivity was 88.64%,specificity was 94.12%,respectively.ConclusionsVTIQ is valuable in differentiating metastatic from non-metastatic cervical lymph nodes.
