椎间隙段椎动脉频谱形态异常对于血管病变的诊断价值

摘　　要：目的探讨椎动脉椎间隙段频谱形态异常对血管病变的诊断价值。方法选择经脑卒中规范化颈部动脉筛查发现椎动脉椎间隙段频谱形态异常,并通过其他影像学检查(CTA、MRA或DSA)证实的154例病例影像资料进行回顾性分析。结果超声诊断"低速低阻"组34例,提示椎动脉起始段或开口处重度狭窄或闭塞,与CTA、MRA或DSA结果比较,诊断符合率为85.29%(29/34);"低速高阻"组44例,提示颅内段椎动脉重度狭窄,诊断符合率79.55%(35/44);"低速单峰"组76例,提示远段椎动脉闭塞,诊断符合率为88.16%(67/76)。结论椎间隙段椎动脉频谱形态异常可以提示其近段、远段病变,为快速寻找病灶部位和严重程度提供重要线索,值得推广应用。Objective To investigate the clinical value of ultrasonography in diagnosing the stenosis of vertebrobasilar vessel by analgsing abnormal pulse Doppler spectrum found in intervertebral artery.Methods 154 cases from conventional stroke screening tests were enrolled for analysis and all subjects were found abnormal pulse Doppler spectrum in intervertebral artery by Ultrasonography.All the results were confirmed by other imaging tests including CT angiography（CTA）,MR angiography（MRA）or Digital Subtraction Angiography（DSA）.Results 34 patients with low velocity and low resistance blood flow in pulse doppler spectrum which suggestive severe stenosis or occlusion of the initial segment of the vertebral artery were confirmed with a diagnosis rate of 85.29%（29/34）.44 patients with low velocity and high resistance blood flow which suggestive severe stenosis of intracranial vertebral artery were confirmed with a diagnosis rate of 79.55%（35/44）.76 patients with low systolic velocity and without diastolic spectrum which suggestive occlusion of distal vertebral artery has a diagnosis rate of 88.16%（67/76）.Conclusions Abnormal pulse Doppler spectrum found in intervertebral artery could help diagnose the proximal and distal vascular stenosis and provide valuable clues to find out the lesion location and severity which worth to be popularized and applied.
