木村病的超声诊断价值

摘　　要：目的探讨木村病(KD)的临床及超声表现,以提高临床与超声医师对该病的认识。方法回顾性分析我院经手术病理证实的木村病患者26例(31个部位)的临床表现、实验室检查以及超声表现。结果男21例,女5例,其中23例外周血嗜酸性粒细胞增多,2例伴肾功能损害。超声表现单发病灶或多个皮下软组织肿物,根据超声图像分为两类型:Ⅰ型表现为肿大淋巴结回声,占15个部位;Ⅱ型表现为团块样或斑片样混合回声,类似"木纹"结构,占16个部位。Ⅰ型的嗜酸性粒细胞百分比、酸性绝对值低于Ⅱ型,差异有统计学意义。结论木村病的超声表现有一定的特征性,对于病变部位的无痛性肿块并伴外周血嗜酸性粒细胞及血清IgE升高者,应考虑木村病的可能。Objective To discuss the clinical and ultrasonic characteristics of Kimura′s disease（KD）,in order to improve clinical and ultrasound doctors′understanding of this disease.Methods Clinical feature,laboratory date and US imaging features of 26 KD（31 parts）proved by operation and pathology in our hospital entered our prospective study.Results Male 21 cases,female 5 cases,among which 23 cases had increased eosinophilic granulocyte in peripheral blood,2 cases were accompanied by kidney damage.US showed single lesion or multiple subcutaneous soft tissue masses.It was divided into two types according to the ultrasonic image：Type I showed enlarged lymph node echo,accounting for 15 parts in the bodies.Type II showed mass or spot piece like mixed echo,structurally similar to ＂ wood grain＂,accounting for 16 parts in their bodies.The percentage of eosinophils and direct eosinophils count in type I were less than in type II,and there were significant differences between the two types.Conclusions KD has some ultrasonic characteristics.When meeting apainless mass and accompanied with increased serum IgE and eosinophilic granulocyte in peripheral blood,we should consider the possibility of KD.
