乌司他丁腹腔灌洗治疗重症胰腺炎的动物实验研究

摘　　要：目的探讨不同方式和不同时期使用乌司他丁腹腔灌洗治疗重症胰腺炎(SAP)大鼠模型的临床效果。方法采用80只雄性Wistar大鼠建立重症胰腺炎(SAP)模型,随机分为假手术组(C)、生理盐水灌洗组(SL)、静脉乌司他丁组(IU)、早期乌司他丁灌洗组(EUL)和晚期乌司他丁灌洗组(LUL),分别给予相应的处理。观察记录各组大鼠术后12 h的生存时间,计算中位生存期。在术后3 h,采集大鼠尾静脉血标本,检测天冬氨酸转氨酶(AST)、丙氨酸转氨酶(ALT)、肌酐(Cr)、尿素(UR)、肌钙蛋白T(Tn T)的含量。结果与C组比较,SL组的生存时间无显著变化;IU组和LUL组虽有一定延长,但差异无统计学意义(P>0.05);而EUL组的中位生存期明显延长,显著长于其他各组(P<0.05)。EUL的各生化指标改善幅度均显著优于其他各组(P<0.01),说明早期乌司他丁灌洗对SAP的治疗效果最佳。其他各组间的比较,总体而言,各指标均有不同程度的改善,但是各组间差异不显著。结论早期乌司他丁腹腔灌洗可纠正SAP模型大鼠的多器官功能损害,显著提高中位生存期。Objective To study the clinical effects of ulinastatin peritoneal lavage in different modes and different periods in the treatment of rat model with severe acute pancreatitis（SAP）.Methods A total of 80 male Wistar rats were adopted for building a SAP model and were randomly divided into a sham-operation group（C）,normal saline lavage group（SL）,intravenous ulinastatin group（IU）,early ulinastatin lavage（EUL） group and late ulinastatin lavage group（LUL）.These groups were provided with corresponding treatment.The survival time of rats 12 h after the operation was recorded to calculate the median survival time.Rat tail venous blood sample was collected three hours after the operation to detect the content of AST,ALT,Cr,UR and T（Tn T）.Results Compared with group C,the survival time of group SL shows no significant change; the survival time of groups IU and LUL was prolonged to some degree,but showed no statistical significance（P〉0.05）; the median survival time of group EUL was obviously prolonged and was much longer than that of other groups（P〈0.05）.All biochemical indicators of group EUL were improved more than those of other groups（P〈 0.01）,indicating that early ulinastatin lavage has the best effect on the treatment of SAP.In general,all indicators of all other groups were improved to some degree,but had no signific ant difference.Conclusion Early ulinastatin lavage can correct multi-organ failures of SAP model rats and greatly prolong the median survival time.
